【摘要】 目的:探讨耳穴埋豆疗法对中医不同证型维持性透析失眠患者的疗效差异。方法:选取维持性透析失眠患者76例,以神门、交感、皮质下、心、肾为主穴,肝、胆、脾、胃、内分泌、耳中为配穴,辨证取穴给予耳穴埋豆疗法,观察患者治疗前后匹兹堡睡眠质量指数量表(PSQI)评分及临床疗效。结果:治疗后,心脾两虚型、心肾不交型患者的PSQI评分较治疗前显著降低,差异有统计学意义(P<0.05)。虚证患者PSQI评分较治疗前显著降低,差异有统计学意义(P<0.05)。虚证患者总有效率为69.49%,高于实证患者47.06%(P<0.05)。结论:耳穴埋豆疗法对心脾两虚、心肾不交型维持性血液透析失眠患者疗效更显著,对虚证患者疗效优于实证患者。更多还原

【Abstract】 Objective:To discuss the therapeutic effect of the method of auricular point pressing with beans on patients undergoing maintenance hemodialysis with insomnia of different Chinese medicine syndrome types.Methods:A total of 76 cases of patients undergoing maintenance hemodialysis with insomnia were involved.Shenmen（TF₄）,sympathesis(AH_6a),subcortex（AT₄）,heart(CO₁₅) and kidney(CO₁₀) were chosen as the main points and liver(CO₁₂),gallbladder(CO₁₁),spleen(C0₁₃),stomach（CO₄）,endocrine(CO₁₈) and ear center（HX₁） as the combination points.Patients were adopted the method of auricular point pressing with beans and the points selected were based on syndrome differentiation.Before and after treatment,the scores of Pittsburgh Sleep Quality Index（PSQI） and the clinical effect were observed.Results:After treatment,the PSQI scores in patients of deficiency of both the heart and spleen type and in patients of disharmony between heart and kidney type were significantly decreased when compared with those before treatment,the difference being significant（P <0.05）.The PSQI score of patients with deficiency syndrome was significantly decreased when compared with that before treatment,the difference being significant（P <0.05）.The total effective rate was 69.49% in patients with deficiency syndrome,higher than that of 47.06% in patients with excess syndrome（P <0.05）.Conclusion:The method of auricular point pressing with beans is more effective in treating patients of deficiency of both the heart and spleen type or patients of disharmony between heart and kidney type undergoing maintenance hemodialysis with insomnia and its effect on patients with deficiency syndrome was better than that on those with excess syndrome.更多还原
