雾化吸入阿米卡星治疗高危铜绿假单胞菌感染的支气管扩张症的疗效观察
Effect observation of aerosolized amikacin in treatment of bronchiectasis with high-risk Pseudomonas aeruginosa infection
【摘要】 目的观察雾化吸入阿米卡星治疗高危铜绿假单胞菌感染的支气管扩张症的效果及可行性。方法将支气管扩张伴高危铜绿假单胞菌感染患者40例随机分为观察组和对照组各20例。观察组给予哌拉西林/舒巴坦钠静脉滴注及阿米卡星雾化吸入,对照组将阿米卡星改为静脉滴注。比较2组的临床疗效及不良反应发生情况。结果观察组总有效率为90.00%高于对照组的60.00%,差异有统计学意义(P<0.05)。观察组肺部啰音明显减少所需要的时间、痰液明显减少所需要的时间和住院时间均短于对照组,差异均有统计学意义(P<0.05)。治疗后2组CPIS评分及CRP均低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05)。观察组有2例轻微恶心呕吐,对照组中有1例发生轻微的恶心呕吐,经处理后可迅速缓解,无后遗症。结论雾化吸入阿米卡星治疗高危铜绿假单胞菌感染的支气管扩张症的疗效较好,安全可靠。
【Abstract】 Objective To observe the effect and feasibility of nebulized amikacin in treatment of bronchiectasis with high-risk Pseudomonas aeruginosa infection.Methods Forty patients with bronchiectasis and high-risk Pseudomonas aeruginosa infection were randomly divided into observation group and control group,each of 20 cases.Observation group was given intravenous infusion of piperacillin/sulbactam and aerosolization of amikacin,and control group was changed to intravenous infusion of amikacin.The clinical efficacy and adverse reactions of two groups were compared.Results The total effective rate of observation group was 90.00% higher than 60.00% of control group,and the difference was statistically significant(P<0.05).The time required for the significant reduction of lung vocalization and sputum and hospitalization time in observation group were shorter than control group,and the difference was statistically significant(P<0.05).After treatment,the CPIS scores and CRP of two groups were lower than those before treatment,and observation group was lower than control group,the difference was statistically significant(P<0.05).There were 2 cases of mild nausea and vomiting in observation group,and 1 case of mild nausea and vomiting in control group,which was quickly relieved after treatment and had no sequelae.ConclusionAerosol inhaled amikacin is effective in treatment of bronchiectasis with high-risk infection of Pseudomonas aeruginosa.
