河北工程大学附属医院院前急救死亡患者特点分析
Analysis of characteristics of pre-hospital first aid dead patients in the Affiliated Hospital of Hebei University of Engineering
【摘要】 目的分析院前急救死亡患者特点,为降低院前病死率提供科学依据。方法收集2016年河北工程大学附属医院丛台院区院前急救患者3 941例,死亡例数为187例,病死率为4.74%。统计分析院前急救死亡患者各年龄段疾病分布、月份分布、呼救时间分布、院前急救时间、院前急救反应半径及各个区域病死率。结果院前急救死亡患者年龄及疾病呈单峰分布,其中老年患者病死率(43.32%)最高,心血管疾病患者病死率(49.73%)最高。院前急救患者9月份病死率最高,呼救发生率最高时间段为6:00~7:59,最低时间段为16:00~17:59。根据患者死亡情况分为死亡组和非死亡组。两组患者调度时间、到达时间、返回时间比较,差异无统计学意义(P>0.05);非死亡组患者现场救治时间短于死亡组(P<0.05)。平均院前急救反应半径(10.26±3.23)km,市区、郊区、农村病死率分别为22.46%、29.95%、47.59%。结论针对院前死亡患者年龄、所患疾病、月份、呼救时间分布及院前急救反应半径等特点,医护人员可进行健康干预,并制定相应措施,以降低院前病死率。
【Abstract】 Objective To analyse the characteristics of pre-hospital first aid dead patients,to proivde scientific basis for reducing pre-hospital morta lity.Methods In 2016,3 941 pre-hospital emergency patients in Congtai District of Affiliated Hospital of Hebei University of Engineering were collected.The death rate was 4.74%.The distribution of disease,month,call time,pre-hospital first aid time,pre-hospital emergency response radius and mortality in each area were analyzed.Results The mortality rate of elderly patients(43.32%)was the highest,and that of cardiovascular patients(49.73%)was the highest.The mortality rate of pre-hospital first aid patients was the highest in September,the highest time of calling for help was 6:00-7:59,and the lowest time was 16:00-17:59.The patients were divided into death group and non-death group according to their mortality.There was no significant differences in the dispatch time,arrival time or return time between the two groups(P>0.05);the on-site treatment time of non-death group was shorter than death group(P<0.05).The average radius of pre-hospital emergency response was(10.26+3.23)km,and the mortality rates in urban,suburban and rural areas were 22.46%,29.95% and 47.59%.Conclusion In view of the characteristics of pre-hospital deaths,such as age,disease,month,time distribution of call and radius of pre-hospital emergency response,medical staff can intervene in health and formulate corresponding measures to reduce pre-hospital mortality.
