颈-股脉搏波传导速度与心脑血管事件的相关性

【摘要】 目的研究自然人群中颈-股脉搏波传导速度(carotid-femoral pulse wave velocity,C-FPWV)与心脑血管事件的关系。方法在包头医学院第二附属医院体检科连续收集的1 153例患者基本资料,并对体检者进行C-FPWV测定,随访9年,失访98例,观察心脑血管事件的发生风险。结果 (1)事件组与非事件组比较年龄、体重指数、收缩压、舒张压、空腹血糖(FBG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、C-FPWV、高血压、糖尿病差异有统计学意义(P<0.01)。(2)Logistic多元回归分析结果表明,年龄、舒张压、高血压、C-FPWV等危险因素与心脑血管事件相关, C-FPWV每增加1 m/s,发生心脑血管事件的危险性增加1.148倍。(3)C-FPWV与心脑血管事件的ROC曲线面积为(0.639±0.022)(95%CI为0.596-0.681,P=0.000),C-FPWV预测心脑血管事件的最佳临界点为PWV=10.25 m/s。结论 C-FPWV是心脑血管事件独立危险因素,C-FPWV值越高心脑血管事件的发生率越高。

【Abstract】 Objective To study the relationship between carotid-femoral pulse wave velocity and cardiovascular and cerebrovascular events.Methods Continuous collection the basic data of 1153 cases of physical examination in Second Affiliated Hospital of Baotou Medical College Medical Branch, and examination conducted by measuring C-FPWV, follow-up of 9 years, lost 98 cases, observation of cardio-cerebral vascular events.Results(1) The difference of age, BMI、SBP、diastolic blood pressure、hypertension、FBG 、TC、TG、LDL-C、Diabetes、C-FPWV between cardio-cerebral vascular events and non-cardio-cerebral vascular events was statistically significant(P<0.01);(2)Logistic regression analysis showed that the risk factors of cardiovascular and cerebrovascular events is age, diastolic blood pressure, C-FPWV, when the C-FPWV increased 1 m/s, the risk of cardiovascular events increased 1.148 times.(3)The ROC curve area of C-FPWV and cardio-cerebral vascular events was 0.639 ± 0.022(95% CI is 0.596-0.681, P=0.000), From ROC Curves plotted between the c-FPWV and cardio-cerebrovascular event,the best cut-off point for predicting cardio-cerebrovascular event were 10.25 m/s.Conclusion C-FPWV is the independent risk factor in cardio-cerebral vascular events, the higher C-FPWV values is, the higher the incidence of cardiovascular events may be.
