吡格列酮联合盐酸多奈哌齐对阿尔兹海默症患者血清中免疫炎症因子、Aβ1-42蛋白和Tau蛋白的影响

【摘要】 目的观察吡格列酮联合盐酸多奈哌齐对阿尔兹海默症(AD)患者血清炎性细胞因子、Aβ1-42蛋白和Tau蛋白水平的影响。方法收集阿尔兹海默症患者86例,分别用二甲双胍联合盐酸多奈哌齐(对照组,n=43)、吡格列酮联合盐酸多奈哌齐(治疗组,n=43)治疗。治疗前、治疗48周后分别对2组患者进行血清血糖、IL-1β、IL-6、TNF-α、Aβ1-42和Tau蛋白的含量进行检测,以及对患者神经功能症状(MMSE和ADL评分)进行评价。结果治疗48周后,2组患者IL-1β、IL-6、TNF-α、Aβ1-42和Tau蛋白水平均低于治疗前,差异无统计学意义(P>0.05);而治疗组血清IL-1β、IL-6、TNF-α、Aβ1-42和Tau蛋白水平显著低于对照组(P<0.05)。结论吡格列酮有效降低阿尔兹海默症患者血清中IL-1β、IL-6、TNF-α、Aβ1-42和Tau蛋白水平,改善患者的神经症状。

【Abstract】 The study was performed to observe the effect of pioglitazone combined donepezil hydrochloride on serum inflammatory cytokines, Aβ1-42 and Tau protein levels in patients with Alzheimer’s disease(AD) and hyperglycemia. Total of 86 patients with Alzheimer’s disease and hyperglycemia were collected and treated with metformin combined with donepezil hydrochloride(control group, n=43) or pioglitazone combined with donepezil hydrochloride(treatment group, n=43). Serum glucose, IL-1β, IL-6, TNF-α, Aβ1-42 and Tau protein levels were measured in both groups before and 48 weeks after treatment; neurological symptoms was evaluate by MMSE and ADL scores. Data showed that 48 weeks after treatment, blood glucose was controlled well in both groups, and the levels of IL-1β, IL-6, TNF-α, Aβ1-42, and Tau protein were lower than those of before treatment. Serum levels of IL-1β, IL-6, TNF-α, Aβ1-42, and Tau protein in the treatment group were significantly lower than those in the control group(P<0.05). After treatment, the neurological symptoms of both groups were improved(P<0.05), and the improvement of the treatment group was significantly better than that of the control group(P<0.05). Taken together, pioglitazone can control blood glucose and improve the patient’s neurological symptoms, while reducing serum levels of IL-1β, IL-6, TNF-α, Aβ1-42 and Tau protein in AD patients with hyperglycemia.
