广泛性子宫切除术对宫颈癌患者下尿路功能的影响研究
Impact of the lower urinary tract dysfunction after radical hysterectomy in patients with cervical cancer
【摘要】 目的通过尿流动力学方法检测宫颈癌患者手术前后下尿路功能改变,评估术后恢复情况。方法宫颈癌患者行广泛性子宫切除术30例,通过对比分析手术前、手术后12~14 d和手术后3个月的最大尿流率、平均尿流率、尿流时间、排尿量、残余尿量、逼尿肌压力、膀胱最大容量、平均逼尿肌压和膀胱顺应性,评估下尿路功能恢复情况。结果手术后12~14 d最大尿流率、平均尿流率、排尿量、膀胱最大容量、平均逼尿肌压和膀胱顺应性与手术前比较均明显下降(P <0.05);手术后3个月最大尿流率显著增加、尿流时间与手术前比较显著延长(P <0.05)。手术后3个月残余尿量与手术前比较明显增加(P <0.05),与手术后12~14 d比较显著下降(P <0.05)。结论通过尿流动力学检查反映出广泛性子宫全切术患者手术后12~14 d排尿和储尿功能较手术前明显下降,手术后3个月逐渐恢复但未达到正常水平。
【Abstract】 Objective To detect the lower urinary tract dysfunction in cervical cancer before and after operation by urine flow dynamic test and evaluate the postoperative recovery. Methods Thirty patients with cervical cancer underwent radical hysterectomy and their recovery of urinary dysfunction was evaluated by comparing the max flow rate, average flow rate, flow time, voided volume, post-void residual and detrusor pressure before operation, 12 —14 days after operation and 3 months after operation. Results The max flow rate,average flow rate, voided volume, residual urine volume, detrusor pressure maximum bladder volume, mean detrusor pressure and bladder compliance significantly decreased on 12 —14 days after operation compared with those before operation(P < 0.05), and the maximum urinary flow rate significantly increased and the time of urinary flow was significantly prolonged at 3 months after operation compared with those before operation(P < 0.05). Post-void residual increased significantly at 3 months after operation and before operation(P < 0.05), decreased significantly at 12 —14 days after operation(P < 0.05). Conclusion The results of the urodynamic examination showed that the micturition function of urination and urine storage of the patients with radical hysterectomy significantly decreased on 12—14 days after operation compared with those before operation, recovered gradually after 3 months but did not reach their normal level.
