甘肃省幽门螺杆菌耐药分析

【摘要】 目的分析甘肃省幽门螺杆菌(HP)临床株对阿莫西林、四环素、甲硝唑、克拉霉素及左氧氟沙星5种常见抗生素的耐药情况。方法纳入需行胃镜检查且近4周未服用抗生素的受试者,体外分离培养HP的临床株,进行E-test药敏试验,检测HP对阿莫西林、四环素、甲硝唑、克拉霉素及左氧氟沙星的最低抑菌浓度。结果 HP对阿莫西林、四环素、甲硝唑、克拉霉素及左氧氟沙星的耐药率分别为2.86%、48.57%、74.29%、54.72%、72.86%。甘肃省HP双重耐药率、三重耐药率、四重耐药率分别为22.64%、32.08%、20.75%。结论 HP对阿莫西林最为敏感,其次为四环素,可作为HP根除方案中选择的抗生素;但HP对克拉霉素、甲硝唑、左氧氟沙星的耐药率相当高,不建议作为HP根除方案中首选的抗生素。

【Abstract】 Objective An investigation was made into the resistance rates of helicobacter pylori(HP) to amoxicillin, tetracycline, metronidazole, clarithromycin and levofloxacin in Gansu Province. Methods HP strains were cultured from the gastric biopsy samples obtained from patients who signed an informed consent and had not taken antibiotics for nearly 4 weeks. The minimum inhibitory concentrations of amoxicillin, tetracycline, metronidazole, clarithromycin and levofloxacin were determined by the E-test method. Results The resistance rates of HP to amoxicillin, tetracycline, metronidazole, clarithromycin and levofloxacin in Gansu Province were 2.86%, 48.57%, 74.29%, 54.72%, 72.86%. The rates of HP dual, three, four drug resistance in Gansu Province were 22.64%, 32.08% and 20.75%, respectively. Conclusion HP was most sensitive to amoxicillin and next to tetracycline, both of which can be used to eradicate HP But the other three medications are very resistant to it and there are not recommended as priority choices.
