急性高血压性脑出血患者血清CCCK-18水平与预后的相关性分析

【摘要】 目的探讨急性高血压性脑出血(AHCH)患者血清gaspase切割的细胞角蛋白18(CCCK-18)水平与预后的相关性分析。方法选取2016年1月至2018年4月在收集佛山市第一人民医院神经外科住院的AHCH患者70例作为观察组,另选取同期健康体检者30例作为对照组,比较两组血清CCCK-18水平,采用酶联免疫吸附法(ELISA)检测患者入院第1天血清CCCK-18水平;3个月后采用改良Rankin量表评分(mRS)评估患者的预后,分为预后良好亚组(mRS≤2分)和预后不良亚组(mRS>2分),采用多因素Logistic回归方法分析影响患者预后的独立危险因素。结果观察组患者血清CCCK-18水平(245.47±52.84)U·L-1明显高于对照组(81.90±35.73)U·L-1,差异有统计学意义(t=15.483,P=0.000)。预后不良亚组NHISS评分、血肿体积、纤维蛋白原、D-二聚体、CCCK-18水平明显高于预后良好亚组,差异有统计学意义(P <0.05)。Logistic回归分析显示,NIHSS评分、血清CCCK-18是影响脑出血患者预后的独立危险因素。结论血清CCCK-18与AHCH患者的预后存在相关性。

【Abstract】 Objective To investigate the correlation analysis of serum caspase-cleaved cytokeratin-18(CCCK-18) level and prognosis of patients with acute hypertensive cerebral hemorrhage(AHCH). Methods A total of 70 patients with AHCH enrolled in this study from January 2016 to April 2018 as observation group,and 30 healthy subjects were selected as control group. Serum CCCK-18 levels were compared between the two groups.Enzyme-linked immunosorbent assay(ELISA) was used to detect the level of serum CCCK-18 on the 1 st day after admission. According to the modified Rankin scale(mRS) at Three months after admission, patients were divided into good prognosis group(mRS ≤ 2 points) and poor prognosis group(mRS > 2 points). Multivariate Logistic regression was used to analyze independent risk factors affecting prognosis. Results The serum CCCK-18 level in the observation group(245.47±52.84)U·L-1 was significantly higher than that in the control group(81.90±35.73)U·L-1, and the difference was statistically significant(t=15.483, P=0.000). The NHISS score, hematoma volume,fibrinogen, D-dimer and CCCK-18 levels in the subgroup with poor prognosis were significantly higher than those in the subgroup with good prognosis(P < 0.05). Logistic regression analysis showed that NIHSS score and serum CCCK-18 were independent risk factors for the prognosis of patients with cerebral hemorrhage. Conclusions There is a correlation between serum CCCK-18 and the prognosis of patients with AHCH.
