【摘要】 目的探讨乳腺肿瘤非整形保乳术与整形保乳术对早期乳腺癌的临床治疗疗效。方法选取本院于2017年3月-2018年6月收治的130例乳腺癌患者作为研究对象,按照具体术式将其分为常规保乳术SBCT(standard breast conservation therapy,SBCT)组65例及整形保乳术OBCT(Oncoplastic breast conserving therapy,OBCT)组65例,SBCT组患者采用传统非整形保乳术治疗,OBCT组患者采用整形保乳术治疗,对比两组患者临床疗效。结果 SBCT组较OBCT组患者手术时间[(143.3±45.4)min VS(146.8±44.9)min]与术中出血量[(149.6±45.7)mL VS(151.7±45.3)mL]均无统计学意义(P>0.05);OBCT组患者术后乳房美容效果优良率较SBCT组(66.15%VS 29.23%,P<0.05);OBCT组术后并发症发生率中血肿及切口愈合不良发生率较SBCT组,差异有统计学意义(t=18.846、5.614、5.849,P<0.05),OBCT组术后并发症血清肿不良发生率较SBCT组(t=18.846,P<0.001);两组患者术后均未发生乳头坏死、上臂水肿情况,差异无统计学意义;比较两组切口裂开及切口感染的发生率情况,差异均无统计学意义(P>0.05)。结论治疗早期乳腺癌中乳腺肿瘤整形保乳术的临床效果明显优于非整形保乳术,整形保乳术安全性高且有良好的乳房美容效果,是值得推荐的临床术式。更多还原

【Abstract】 Objective To explore the clinical efficacy of non-plastic breast conserving surgery and plastic breast conserving surgery for early breast cancer.Methods 130 patients with breast cancer admitted to our hospital from March 2017 to June 2018 were selected as the research objects.They were divided into standard breast conservation therapy(SBCT)group(65 cases)and plastic breast conserving therapy(OBCT)group(65 cases).The SBCT group was treated with conventional non-plastic breast conserving therapy,while the OBCT group was treated with conventional non-plastic breast conserving therapy.Plastic breast-conserving surgery was used to compare the clinical efficacy of the two groups.Results There was no significant difference between SBCT group and OBCT group in operation time[(143.3 soil 45.4)min VS(146.8±44.9)min]and intraoperative bleeding volume[(149.6±45.7)mL VS(151.7±45.3)mL](P>0.05);the excellent and good rate of breast cosmetic effect in OBCT group was 66.15% VS 29.23%,P<0.05);the incidence of hematoma and poor wound healing in OBCT group was higher than that in SBCT group.Statistical significance(t=18.846,5.614,5.849,P<0.05).The incidence of serum edema in OBCT group was higher than that in SBCT group(t=18.846,P<0.001).No nipple necrosis or upper arm edema occurred in both groups after operation.There was no significant difference in the incidence of incision dehiscence and infection between the two groups(P>0.05).Conclusion The clinical effect of breast tumor shaping and breast conserving surgery in early breast cancer is obviously better than non-plastic breast conserving surgery.The plastic breast conserving surgery has high safety and good breast cosmetic effect,which is a recommended clinical procedure.更多还原
