丘脑梗死患者记忆障碍的初步研究
Memory impairment in thalamic infarction patients
【摘要】 目的研究丘脑梗死患者发病3个月时的认知功能损害,进一步探讨基于事件的前瞻性记忆(event-based prospective memory,EBPM)和基于时间的前瞻性记忆(time-based prospective memory,TBPM)损害特征。方法选择2016年1月~2018年7月无锡市第二人民医院神经内科门诊和住院的丘脑梗死患者31例(丘脑梗死组),收集我院同期在性别、年龄和受教育程度与丘脑梗死患者相匹配的健康体检者30例(对照组)。采用简易智能状态检查量表和中文剑桥前瞻性记忆测试量表对受试者进行测试。结果丘脑梗死组时间定向、注意与计算、延迟记忆、言语重复、三步命令和结构能力评分明显低于对照组,差异有统计学意义(P<0.05,P<0.01)。丘脑梗死组TBPM评分明显低于对照组,差异有统计学意义[(9.27±1.81)分vs (13.66±2.74)分,P<0.01)]。而2组EBPM评分比较,差异无统计学意义[(13.40±1.57)分vs (13.85±1.91)分,P>0.05]。左侧丘脑梗死与右侧丘脑梗死患者TBPM、EBPM评分比较,差异无统计学意义[(9.60±1.84)分vs (10.09±1.54)分,(14.57±1.73)分vs (13.74±1.94)分,P>0.05]。结论丘脑梗死患者发病3个月时存在认知障碍,提示延时记忆和TBPM仍受到损害,EBPM却保持相对正常。
【Abstract】 Objective To study the cognitive impairment in thalamic infarction patients at month 3 after onset of thalamic infarction,and the characteristics of cognitive impairment according to event-based prospective memory(EBPM)and time-based prospective memory(TBPM).Methods Thirty-one thalamic infarction patients admitted to our hospital from January 2016 to July 2018 served as a thalamic infarction group and 30 healthy adults who underwent physical examination served as a control group.Their cognitive impairment was scored according to the MMSE and MoCA Scales respectively.Results The scores of time orientation,attension,calculation,delayed memory,speech repetition,three-stage command and structure ability were significantly lower in thalamic infarction group than in control group(P<0.05,P<0.01).The TBPM score was significantly lower in thalamic infarction group than in control group(9.27±1.81 vs 13.66±2.74,P<0.01).No significant difference was detected in EBPM score between the two groups(13.40±1.57 vs13.85±1.91,P>0.05)and in TBPM and EBPM scores of left and right thalamic infarction patients(9.60±1.84 vs 10.09±1.54,14.57±1.73 vs 13.74±1.94,P>0.05).Conclusion Cognitive impairment occurs in thalamic infarction patients at month 3 after onset of the disease,which indicates that their delayed memory and TBPM are impaired while their EBPM remains relatively normal.
