短程持续非卧床腹膜透析患者血清瘦素的特点分析

【摘要】 目的 探讨短程持续非卧床腹膜透析(CAPD)患者血清瘦素(LEP)及相关指标的临床特点。方法 选取CAPD在3~36个月时间段的患者65例,根据透析时长6个月、1、2、3年等4个节点分为A组(≤6个月,7例)、B组(6~12个月,16例)、C组(12~24个月,21例)、D组(24~36个月,21例)4组,记录患者一般临床指标如性别、年龄(岁)、身高(cm)、体重(kg)、体重指数(BMI)等一般指标,并留取患者清晨空腹静脉血,检测各项血液生化指标,用ELISA检测血清瘦素(LEP)浓度,并进行相关统计学分析。选取根据K/DOQI指南诊断慢性肾脏病Ⅴ期,尚未行透析患者24例为对照组。结果 Pearson相关性分析显示,LEP水平与BMI呈正相关(r=0.267,P=0.03<0.05),透析时长与血TC水平存在负相关(r=-0.286,P=0.021<0.05),Alb与Hb水平存在正相关(r=0.293,P=0.02<0.05)。根据独立样本Kruskal-Wallis检验,B组、C组、D组随着透析时长增加,PD患者(除外PD≤6个月)血中LEP水平有下降趋势。结论 本腹透中心在透程3年内67例PD患者总体营养状况良好,而PD>6个月患者血清LEP浓度随着透析时长递增有降低,PD患者自身随着透析时长增加血清LEP水平是否显著性变化有待进一步研究。更多还原

【Abstract】 Objective To investigate the clinical characteristics of serum leptin(LEP)in short-term continuous ambulatory peritoneal dialysis(CAPD)patients.Methods The CAPD in 3 ~36 months period,65 cases of patients,according to the dialysis duration of 6 months,1,2,3 years and 4 nodes were divided into group A(6 months or less,7 cases)and group B(6 to 12 months,16 cases),group C(12 to 24 months,21 cases),group D(24 to 36 months,21 cases were four groups,records of patients general clinical indicators such as gender,age(years old),height(cm)and weight(kg),body mass index(BMI),such as general indicators,and to return patients early in the morning fasting venous blood,our hospital clinical laboratory testing the blood biochemical indexes,Serum leptin(LEP)concentration was detected by ELISA,and the correlation was statistically analyzed.Patients with stage 5 chronic kidney disease diagnosed according to K/DOQI guidelines and not on dialysis were selected as the control group(24 cases).Results Pearson correlation analysis showed that LEP level was positively correlated with BMI(r=0.267,P=0.03<0.05),duration of dialysis was negatively correlated with TC level(r=-0.286,P=0.021<0.05),and Alb was positively correlated with Hb level(r=0.293,P=0.02<0.05).According to Kruskal-Wallis test of independent samples,LEP level in the blood of PD patients(except PD<6 months)decreases with the increase of dialysis duration in groups B,C and D.Conclusion The overall nutritional status of 67 PD patients in this dialysis center is good within 3 years after dialysis,while serum LEP concentration of PD patients at 6 months after PD>decreased with the increase of dialysis duration.Whether serum LEP level of PD patients changes significantly with the increase of dialysis duration remains to be further studied.更多还原
