症状性颅内动脉狭窄患者的血小板参数变化特点及临床意义

【摘要】 目的 探讨症状性颅内动脉狭窄患者血小板参数的变化特点及临床意义。方法 回顾性分析143例颅内动脉中重度狭窄患者的临床资料。按既往是否发生过缺血性脑卒中事件分为症状性颅内动脉狭窄组及无症状性颅内动脉狭窄组。记录两组患者的一般资料及血小板参数,即血小板计数(PLT)、血小板压积(PCT)、平均血小板体积(MPV)、血小板分布宽度(PDW)。单因素分析两组的各项血小板参数,进一步多因素Logistic回归分析血小板参数对症状性颅内动脉狭窄的影响。结果 症状性颅内动脉狭窄组(n=66)MPV、PDW均大于无症状性颅内动脉狭窄组(P<0.01或P<0.05),而两组PLT、PCT比较差异均无统计学意义。多因素Logistic回归分析显示MPV(OR=2.697,95%CI 1.874~3.881,P=0.000)、PDW(OR=1.156,95%CI 1.047~1.277,P=0.004)是症状性颅内动脉狭窄的独立危险因素。结论 血小板参数MPV、PDW增大,提示症状性颅内动脉狭窄发生的风险高。

【Abstract】 Objective To observe the changes of platelet parameters in patients with symptomatic intracranial arterial stenosis,expect to obtain the objective indicators for its identification.Methods A retrospective study was conducted,clinical date from 143 patients with moderate and severe intracranial arterial stenosis were collected.According to previous ischemic stroke events,the patients were divided into symptomatic intracranial artery stenosis group and asymptomatic intracranial artery stenosis group.The clinical general data and platelet parameters of the two groups were recorded,including platelet count(PLT),plateletcrit(PCT),mean platelet volume(MPV),and platelet distribution width(PDW).The platelet parameters of the two groups were analyzed by univariate analysis,the effect of platelet parameters on symptomatic intracranial artery stenosis was estimated using logistic regression model.Results Both MPV and PDW in symptomatic intracranial artery stenosis group(n=66)were significantly higher than that in asymptomatic intracranial artery stenosis(P<0.01 or P<0.05).There was no significant difference in PLT,PCT between the two groups.Logistic regression analysis showed that MPV(OR=2.697,95%CI 1.874-3.881,P=0.000).PDW(OR=1.156,95%CI 1.047-1.277,P=0.004)were independent risk factors for symptomatic intracranial artery stenosis.Conclusion Increased platelet parameters MPV and PDW suggest a high risk of symptomatic intracranial artery stenosis.
