1.5TMRI成像单指数模型及体素内不相干运动诊断胰腺癌的应用价值

【摘要】 目的 分析1.5T MRI弥散加权成像(DWI)单指数模型及体素内不相干运动成像(IVIM)诊断胰腺癌的应用价值。方法 选择2015年10月至2017年6月经病理检查确诊的胰腺癌患者46例为胰腺癌组,选择同期胰腺无异常的健康者50例为健康组。分析两组的常规单b值DWI序列生成单指数模型ADC值、ADCstandard、双阶单指数相关扩散系数值(D)、灌注相关扩散系数值(D*)、灌注分数(f)值。结果 胰腺癌组与健康组的T2WI、DWI分布比较,差异有统计学意义(P<0.05);而T1WI分布,差异无统计学意义(P>0.05)。胰腺癌组的ADC、ADCstandard、D、Df参数值低于健康组,差异有统计学意义(P<0.05)。胰腺癌组的ADC、D、Df参数值鉴别胰腺癌有一定的诊断效能,ADCstandard、D特异度较高,D的灵敏度较高。结论 1.5TMRIDWI单指数模型及IVIM单指数模型有一定诊断胰腺癌的临床价值,同时获得较全面的参数信息,灵敏度与特异性较高。

【Abstract】 Objective To analyze the value of 1.5 T MRI diffusion weighted imaging( DWI) single index model and voxel incoherent motion imaging(IVIM) in the diagnosis of pancreatic cancer. Methods 46 patients with pathologically confirmed pancreatic cancer in our hospital from October 2015 to June 2017 were selected as pancreatic cancer group. 50 healthy patients without abnormal pancreas at the same time were selected as healthy group. Normal single-b value DWI sequences from two groups generated single-exponential model ADC values, ADC standard, dual-order single exponential correlation diffusion coefficient values(D), perfusion-related diffusion coefficient values(D*), perfusion scores(f) value were analyzed. Results The differences in T2WI and DWI distributions between pancreatic cancer and healthy groups were statistically significant(P<0.05),but there was no significant difference in T1WI distribution(P>0.05). The values of ADC, ADC standard, D, D*, and f in pancreatic cancer group were lower than those in healthy group(P<0.05). The ADC, D, D*, and f parameters in the pancreatic cancer group had certain diagnostic efficacy for the differential diagnosis of pancreatic cancer.’The specificity of ADC-standard and D* were higher, and the sensitivity of D was higher.Conclusion The 1.5 T MRI DWI single index model and the IVIM single index model have certain clinical value in the diagnosis of pancreatic cancer.At the same time, more comprehensive parameter information is obtained, which has high sensitivity and specificity.
