【摘要】 目的探讨后路椎弓根钉固定经伤椎椎弓根植骨治疗老年胸腰段椎体压缩性骨折的临床疗效。方法采用椎弓根钉固定经伤椎椎弓根植骨治疗25例老年胸腰段椎体压缩性骨折患者。比较术前、术后2周和末次随访时的疼痛VAS评分、伤椎前缘高度比、矢状位Cobb角。结果患者均获得随访,时间12～16 (14. 6±1. 2)个月。伤口均一期愈合,均无断钉、断棒现象,末次随访椎体高度未见明显丢失。术后2周及末次随访时VAS评分、伤椎前缘高度比、矢状位Cobb角均较术前改善,差异均有统计学意义(P <0. 05)。Frankel分级:6例术前D级者末次随访均恢复为E级; 19例术前E级者无神经功能损伤。结论后路椎弓根钉固定经椎弓根植骨治疗老年胸腰段椎体压缩性骨折安全可靠,疗效满意。更多还原

【Abstract】 Objective To investigate the effect of posterior pedicle fixation combined with transpedicle bone grafting in the treatment of elderly thoracolumbar fracture. Methods The 25 cases of elderly thoracolumbar fracture were treated with posterior pedicle fixation combined with transpedicle bone grafting. The VAS,the ratio of anterior height of injured vertebrae and the sagittal Cobb angle of injured vertebrae were assessed at the preoperation and postoperative 2 weeks,and final follow-up. Results The patients were followed up for 12 ～ 16( 14. 6 ± 1. 2) months. All patients’ wound healed in the one stage,without infection and rod broken,and there was no obvious loss of the injured vertebrae height at last follow-up. At the preoperation and postoperative 2 weeks,final follow-up,the VAS,the ratio of injured vertebrae anterior height and the sagittal Cobb angle of injured vertebrae were improved after surgery compared with the preoperation,all these differences were statistically significant( P < 0. 05). Frankel grade: 6 cases of preoperative D grade improved to E grade at the final follow-up,19 cases of E grade had no neurological injuries after operation. Conclusions The elderly patients with thoracolumbar compression fracture are treated with posterior pedicle screw fixation and transpedicle bone grafting,the outcome is confirmed safely and effective.更多还原
