【摘要】 目的了解陪伴参与护理工作对髋关节置换患者快速康复的影响,探索术后患者快速康复的护理策略。方法采用Harris髋关节功能评分表、SF-36量表、WHO手卫生观察表结合自制问卷量表,在2016年4月至2018年3月期间对髋关节置换术患者进行调查,并收集相关数据。结果陪伴参与护理工作模式与传统模式医务人员人员在手卫生执行方面(χ~2=7.189,P=0.07)、患者术后肺炎发生(χ~2=4.143,P=0.042)、下肢深静脉血栓发生(χ~2=4.425,P=0.023)、患者住院时间(t=-2.778,P=0.006)、患者在出院时生命质量(t=19.575,P<0.001)与Harris评分(t=0.221,P<0.001)、患者对参与组与对照组护理工作认可程度(Z=-2.711,P=0.007)差异有统计学意义,而医务人员手卫生方法正确率(χ~2=0.038,P=0.845)、术后1周患者生命质量(t=-1.956,P=0.052)差异无统计学意义。结论陪伴参与护理工作的模式可以使髋关节置换患者快速康复,提高医务人员手卫生执行率,改善患者的生命质量,缩短住院时间,减少肺炎及下肢深静脉血栓的发生,对陪伴进行干预内容还需要进行细化。更多还原

【Abstract】 Objective To learn the effects on rapid rehabilitation of hip replacement patients by accompany and care management and to explore nursing strategies for patients with rapid postoperative rehabilitation. Methods Harris Hip Function Scale, SF-36 Scale, WHO Hand Hygiene Observation Scale and Self-made Questionnaire Scale were used to investigate hip replacement patients from April 2016 to March 2018,and relevant data were collected. Results Statistically significant differences were found in medical personnel performing hand hygiene(χ~2=7.189,P = 0.07), postoperative pneumonia(χ~2 = 4.143, P = 0.042), lower extremity deep venous thrombosis(χ~2= 4.425, P = 0.023), time spent in hospital(T =-2.778, P = 0.006), quality of life at discharge(T = 19.575, P < 0.001),and Harris scores when discharged(T = 0.221, P < 0.001),recognition of care work(Z =-2.711,P = 0.007). However, no statistically significant differences were found in the accuracy of manual hygiene methods of medical personnel(χ~2= 0.038,P = 0.845),quality of life after operation in one week(T=-1.956, P = 0.052)etc. Conclusion By accompanying and care mangement, patients with hip replacement could recover more quickly,the rate of hand hygiene with medical staff could be improved, the quality of life of patients could be improved, the time of hospitalization could be shortened, and the occurrence of pneumonia and lower extremity deep venous thrombosis could be reduced. However, clear and detailed introcuction of accompany intervention are in need.更多还原
