T-SPOT与结核菌素实验对结核病患者的临床诊断价值分析

【摘要】 目的分析T-SPOT与结核菌素实验对结核病患者的临床价值。方法选取2015年12月~2017年12月疑似结核病患者202例,经过实验组织学检验100例患者确诊为结核病患者,进行外周血T-SPOT检测和结核菌素实验,比较两组患者诊断结果。结果以最终实验组织学检验为金标准,T-SPOT阳性检出率明显优于结核菌素检出率,差异具有统计学意义(P<0.05),T-SPOT监测灵敏度、特异度、准确度均优于结核菌素实验,差异具有统计学意义(P<0.05),T-SPOT检测Kappa值为0.985,说明一致性良好,结核菌素实验检测Kappa值为0.512,说明两者一致性中等。结论利用T-SPOT诊断结核病患者,敏感度高且特异性明显,对结核和肺外结核都有很好的应用价值,值得推广应用。

【Abstract】 Objective To analyze the clinical value of T-SPOT and tuberculin test for tuberculosis patients.Methods 202 suspected tuberculosis patients from December 2015 to December 2017 were selected and those100 confirmed tuberculosis patients were examined by histology. T-SPOT test and tuberculin test for peripheral blood were carried out to compare the diagnostic results between the two groups. With the final histological test as the gold standard,the positive detection rate of T-SPOT was significantly better than that of tuberculin bacteria(P<0.05). The sensitivity,specificity and accuracy of T-SPOT surveillance were better than that of tuberculin test(P < 0.05). The difference was statistically significant(P <0.05). Results The Kappa value of T-SPOT surveillance was 0.985,indicating good consistency,tuberculin test Kappa value of 0.512,indicating that the consistency between the two is medium. Conclusion T-SPOT was used to diagnose tuberculosis patients with high sensitivity and obvious specificity,which had good application value for tuberculosis and extrapulmonary tuberculosis and was worthy of promotion and application.
