【摘要】 目的观察温阳益气方对乳腺癌术后患者生活质量的影响和中医症状改善的临床疗效。方法将60例乳腺癌术后阳气虚证患者随机分为治疗组30例、对照组30例,剔除脱落病例后治疗组28例、对照组29例。在口服第三代芳香化酶抑制剂的基础上,治疗组口服温阳益气方,对照组口服安慰剂。观察2组健康调查简表(SF-36)评分、乳腺癌患者生命质量测定量表(FACT-B)评分、中医证候疗效、中医症状评分。结果 2组SF-36量表评分比较,治疗后治疗组一般健康状况、生理机能、躯体疼痛、精力、社会功能、心理健康评分较本组治疗前均提高(P<0.05)。对照组躯体疼痛、精力评分较本组治疗前均提高(P<0.05),治疗组一般健康状况、生理机能、精力、社会功能评分较对照组提高明显,治疗前后评分差值比较,差异均有统计学意义(P<0.05)。2组FACT-B量表评分比较,治疗后治疗组社会/家庭、功能状况、生理状况、个人情感、附加关注评分较本组治疗前均提高(P<0.05)。对照组附加关注评分较本组治疗前提高(P<0.05)。治疗组社会/家庭、功能状况、生理状况、个人情感评分较对照组提高明显,治疗前后评分差值比较,差异均有统计学意义(P<0.05)。2组中医证候疗效比较,治疗后治疗组28例中,显效9例,有效13例,无效6例,总有效22例,总有效率为78.57%;对照组29例中,显效0例,有效19例,无效10例,总有效19例,总有效率为65.52%。经秩和检验,Z=-4.895,P<0.001,说明治疗组中医证候疗效优于对照组。2组中医症状评分比较,治疗后治疗组神疲乏力、多汗、小便清长、大便溏薄、失眠评分较本组治疗前均减少(P<0.05),对照组大便溏薄评分较本组治疗前减少(P<0.05)。治疗组神疲乏力、多汗、小便清长、大便溏薄、失眠评分较对照组减少明显,治疗前后评分差值比较,差异均有统计学意义(P<0.05)。结论温阳益气方能提高乳腺癌术后患者生活质量,改善乳腺癌术后阳气虚的相关症状,具有较好的临床疗效。更多还原

【Abstract】 Objective To observe the therapeutic efficiency of Wenyang Yiqi Decoction on life quality and the TCM symptoms of patients with breast cancer after operation. Methods 60 patients with breast cancer after operation were randomly divided into treatment group and control group, with 30 in each. After removing the shedding cases, there were 28 cases in treatment group while 29 cases in control group. On the basis of oral third generation aromatase inhibitors,the treatment group was treated with Wenyang Yiqi Decoction,and the control group was given placebo. SF-36, FACT-B, the TCM syndromes and the TCM symptoms were observed before and after treatment. Results The SF-36: the scores of general health, physiological function, physical pain, energy, social function and mental health in the treatment group were higher than before(P<0.05); the scores of physical pain and energy in the control group were higher than before(P<0.05); and the scores of general health, physiological function, energy and social function in the treatment group were higher than control group(P<0.05). The difference of scores before and after treatment was statistically significant(P<0.05). The FACT-B: the scores of social/family, functional status, physiological status, personal emotions and additional concerns in the treatment group were all higher than before(P<0.05); the scores of additional concerns in the control group were higher than before(P<0.05); the scores of social/family, functional status, physiological status and personal emotions in the treatment group were significantly higher than control group, the scores of additional concerns in the treatment group were significantly higher than control group. The difference of scores before and after treatment was statistically significant(P<0.05). TCM syndrome curative effect: In the treatment group,there were 9 cases with obvious effects, 13 showed effective, 6 showed ineffective, and the total effective rate was 78.57%. In the control group there was 19 showed effective, 10 showed ineffective, and the total effective rate was 65.52%. Rank sum test analysis showed Z=-4.895, P<0.001, which indicated treatment group was superior to control group. TCM symptoms: the scores of fatigue, sweating, long urine, thin stool and insomnia in the treatment group were lower than those before(P<0.05), while thin stool in the control group was lower than before(P<0.05). The scores of fatigue, sweating, long urine, thin stool and insomnia in the treatment group were significantly lower than control group, the difference of scores before and after treatment was statistically significant(P<0.05). Conclusion Wenyang Yiqi Decoction can improve life quality and the symptoms related to yang deficiency after breast cancer operation, which means a good clinical effect.更多还原
