非手术脊柱减压系统治疗腰椎间盘突出症的疗效观察及表面肌电图分析
Clinical observation and surface electromyography analysis of non operative spinal decompression system in the treatment of lumbar disc herniation
【摘要】 目的探讨非手术脊柱减压系统治疗腰椎间盘突出症(lumbar disc herniation,LDH)的临床疗效,并通过表面肌电图分析评价其对椎旁肌肉的影响。方法选择2017年1月~2018年1月于本院治疗的68例LDH患者,采用随机数字表法分为两组:观察组34例,予以非手术脊柱减压系统治疗;对照组34例,采用常规电动牵引床治疗。两组治疗周期均为1次/d,5次/周,连续4周。分别于治疗前后进行VAS评分、JOA评分和表面肌电图评价。结果 (1)疗效观察:两组治疗后,VAS评分均较治疗前显著降低(P<0.05),JOA评分均有显著提高(P<0.05),且观察组改善效果显著优于对照组,组间差异均有统计学意义(P<0.05)。(2)表面肌电图分析:两组治疗后,患侧腰部竖脊肌、多裂肌的平均振幅和斜率均显著升高(P<0.05),但观察组治疗后的竖脊肌、多裂肌平均振幅和斜率均显著高于对照组,差异均有统计学意义(P<0.05)。结论 LDH患者采用非手术脊柱减压系统治疗,能够有效减轻患者疼痛感,提高生活质量,且有助于改善椎旁肌肉肌力,促进局部的生物力学平衡,其疗效优于常规牵引。
【Abstract】 Objective To explore the clinical efficacy of non operative spinal decompression system in the treatment of lumbar disc herniation( LDH),and its influence on paraspinal muscles was evaluated by surface electromyography. Methods From January 2017 to January 2018,68 cases of LDH who were treated in our hospital were divided into two groups by random digital table method,with 34 cases in each group. The observation group was treated with non operative spinal decompression system,and the control group was treated with conventional electric traction bed. The two groups were treated for 1 time/d,5 times/week for 4 weeks. The VAS score,JOA score and surface electromyography were evaluated before and after treatment. Results( 1) Observation of curative effect: after treatment,the VAS score of the two groups was significantly lower than before treatment( P<0.05),and the JOA score was significantly improved( P<0.05),and the improvement effect of the observation group was significantly better than that of the control group,the difference between the two groups was statistically significant( P < 0. 05).( 2) Surface electromyogram analysis: after treatment,the average amplitude and slope of the affected lumbar erector spine muscle and polyfidus muscle were significantly increased in both two groups( P<0.05),but the average amplitude and slope of the observation group were significantly higher than those of the control group( P<0.05).Conclusion Treatment of LDH patients with non operative spinal decompression can effectively reduce pain and improve the quality of life,and help to improve the muscle strength of the partebrovertebral muscles and promote the local biomechanical balance. The effect is better than conventional traction.
