【摘要】 目的分析神经科患者发生院内泌尿系感染的临床及病原学特点并探讨预防控制的方法和策略。方法对天津市环湖医院2014年1月1日—2017年12月31日出院的91 999例患者进行回顾性院内泌尿系感染临床分析和致病菌株菌种鉴定,并进行统计学分析。结果神经内外科患者发生院内泌尿系感染累计481例,泌尿系感染率为0.52%,其中神经内科院内泌尿感染率0.46%,神经外科0.63%,神经内外科院内泌尿系感染发病率差异有统计学意义(P <0.01)。年龄、住院天数、是否插尿管、意识朦胧、气管插管和糖尿病在神经内神经外科的院内泌尿感染患者中分布的差异有统计学意义(P <0.01)。院内泌尿系感染病原菌以革兰氏阴性菌为主,主要为大肠埃希氏菌。结论神经外科院内泌尿系感染率较高,可能与易感因素相对较多有关,应采取综合防治措施,降低患者院内泌尿系感染的发生。更多还原

【Abstract】 Objective The study aimed to analyze clinical and pathogenic characteristics of nosocomial urinary tract infections in neurology-related patients, so as to enlighten ways to prevent the occurrence of the urinary tract infections. Methods We retrospectively analyzed 91 999 cases from 1 st January 2014 to 31 st December 2017 in Tianjin Huanhu Hospital. Clinical analysis and identification of pathogenic strains were carried out and analyzed by statistics. Results 481 cases were infected and the rate of nosocomial urinary tract infections was 0.52%, while 0.46% in neurology departments and 0.63% in neurosurgery departments. The difference of infection rate between the neurology and neurosurgery departments was statistically significant(P < 0.01). Age, length of stay, urethral catheter, haziness, tracheotomy, and diabetes were significantly different in nosocomial urinary tract infections between two kinds of departments. And the major pathogenic bacteria were Gram-negative bacill, mainly E. coli. Conclusion The higher infection rate is observed in neurosurgery departments. Patients in neurosurgery departments are more susceptible to infections, precautionary measures should be taken to reduce the rate of nosocomial urinary tract infections.更多还原
