18F-FDG PET/CT对继发性嗜血细胞综合征病因诊断分析

【摘要】 目的分析18F-FDG PET/CT对继发性噬血细胞综合征原发病因诊断的价值。方法回顾性分析16例继发性噬血细胞综合征患者的18F-FDG PET/CT影像表现及代谢特征,并将PET/CT诊断与临床确诊结果进行比较分析。结果 18FFDG PET/CT诊断8例恶性肿瘤,其中5例确诊为淋巴瘤、1例确诊多发性骨髓瘤、1例确诊结核、1例确诊成人still病;诊断6例感染性病变,其中4例确诊为感染(2例EB病毒感染、1例细菌感染、1例结核),1例确诊为淋巴瘤,1例病因未明确;诊断1例关节炎,临床确诊为类风湿关节炎;1例PET/CT无异常表现。结论 18F-FDG PET/CT对明确继发性HPS的病因尤其是鉴别原发病的良恶性方面具有较大价值,在临床诊治策略制订中具有重要意义。

【Abstract】 Objective To investigate the18 F-FDG PET/CT findings of secondary hemophagocytic syndrome(HPS),and to analyze the value of etiological diagnosis. Methods A retrospective study was conducted on the18 F-FDG CT findings of 16 cases of secondary HPS,then all the18 F-FDG PET/CT diagnosis and the clinical diagnosis result were comparative analysed. Results Of all the 16 patients,8 cases were diagnosed in malignancy by18 F-FDG PET/CT,among them 5 were diagnosed as non-Hodgkin’s lymphoma,one was diagnosed as multiple myeloma,one was tuberculosis and another was adult onset still disease;6 cases were diagnosed infectious disease by PET/CT,among them 4 case diagnosed as infectious disease(including 2 EBV infection、1 Bacterial infection and 1 tuberculosis),1 case diagnosed as lymphoma,1 case etiology undetermined;1 case arthritis which was diagnosed as rheumatoid arthritis;1 case showed normal in PET/CT. Conclusion18 F-FDG PET/CT is great value in diagnosing primary disease of secondary Hemophagocytic Syndrome,especially in differetiating benign and malignant lesions. It is of great significance in making clinical diagnosis and treatment strategy.
