BOLD-fMRl联合DTI在运动区脑肿瘤切除术中的应用
Application of BOLD-fM Rl combined with DTI in brain tumor surgery involving motor cortex
【摘要】 目的 :探讨BOLD-f MRI联合DTI在运动区脑肿瘤术前定位运动功能区及皮质下纤维束中的可行性。方法 :收集38例运动区脑肿瘤患者,随机分为试验组(术前联合应用BOLD-f MRI、DTI)20例和对照组(术前应用DTI)18例。2组均行运动区脑肿瘤神经导航术。使用SPSS 22.0软件分析2组手术效果及预后情况。结果:试验组肿瘤全切率(80.00%,16/20)高于对照组(50.00%,9/18)(P> 0.05);试验组术后致残率(30.00%,6/20)低于对照组(66.67%,12/18)(P<0.05)。结论:BOLD-f MRI联合DTI可帮助运动区脑肿瘤的术前功能区定位,以及设计手术入路,从而更好地保护患者的皮质下纤维束和运动功能区。
【Abstract】 Objective:To investigate the feasibility of using BOLD-f MRI to locate motor function areas and the use of DTI to display subcortical fiber bundles before brain tumors surgery in motor cortex. Methods:A total of 38 patients with brain tumors in motor cortex were admitted to the test group by DTI combined with BOLD imaging navigation(n= 20) and the control group by only DTI navigation(n= 18). The patients in the two groups underwent brain tumor neuronavigation surgery. The surgical outcome and prognosis of the two groups were statistically analyzed using SPSS 22.0. Results:The total tumor resection rate was50.00% in the control group,which was lower than that in the test group(P> 0.05). The postoperative morbidity rate of the control group was 66.67%,higher than that in the test group(P< 0.05). Conclusion:Preoperative application of BOLD-f MRI and DTI in motor function areas brain tumors can help the surgeon to locate the preoperative functional area and design a surgical approach to protect the patient’s motor function area.
