后侧支撑板固定治疗胫骨后pilon骨折效果及对患者关节功能的影响
Effect of Posterior Support Plate Fixation on Post-pillar Fracture of Humerus and Its Effect on Joint Function
【摘要】 目的探讨后侧支撑板固定治疗胫骨后pilon骨折效果及对患者关节功能的影响。方法方便选取该院2011年1月—2018年4月间收治的42例胫骨后pilon骨折患者,按治疗方式分为2组,对照组20例予以前向后空心钉固定治疗,研究组22例予以后侧支撑板固定治疗。比较两组患者临床疗效、临床相关指标、术后1年美国足踝外科协会(AOFAS)评分、可视化疼痛量表(VAS)、AO影像学评分标准评分以及并发症发生情况。结果研究组优良率(86.36%)显著高于对照组(55.00%)(χ~2=5.049,P<0.05)。研究组住院时间(13.51±3.54)d、手术时间(73.51±10.59)min与对照组(13.50±3.36)d,(75.85±10.53)min比较差异无统计学意义(t=0.784,0.558,P>0.05),研究组骨折愈合时间(15.43±2.75)周、完全负重时间(18.49±3.86周)与对照组(17.98±3.58)周,(20.71±2.46)周比较差异有统计学意义(t=4.026,3.152,P<0.05)。研究组VAS评分(0.81±0.26分)、AO影像学评分(0.52±0.13)分低于对照组[(1.26±0.58)分,(0.66±0.51)分],AOFAS评分(89.51±9.85分)高于对照组(84.64±8.25)分(t=4.256,4.726,2.423,P<0.05)。研究组并发症发生率(4.54%)与对照组(9.09%)比较差异无统计学意义(χ~2=0.469,P>0.05)。结论后侧支撑板固定治疗胫骨后pilon骨折可改善踝关节功能、缓解疼痛,促进关节功能的恢复,缩短骨折愈合时间及完全负重时间。
【Abstract】 Objective To investigate the effect of posterior support plate fixation on the treatment of iliac bone fracture and its effect on joint function. Methods Convenient select forty-two patients with posterior tibiofibular fractures admitted to our hospital from January 2011 to April 2018 were divided into two groups. The control group received 20 cases of anterior and posterior cannulated screw fixation, and the study group received 22 cases of posterior support plate fixation for treatment.The clinical efficacy, clinically relevant indicators, the American Foot and Ankle Surgery Association(AOFAS) score, the Visualized Pain Scale(VAS), the AO imaging score, and the complications were compared between the two groups. Results The excellent rate of the study group(86.36%) was significantly higher than that of the control group(55.00%)(χ2=5.049,P<0.05). There was no significant difference between the study group(13.51±3.54)d and operation time(73.51±10.59)min and the control group [(13.50±3.36)d,(75.85±10.53)min](t=0.784, 0.558, P>0.05). The fracture healing time(15.43±2.75)weeks and the complete weight-bearing time(18.49±3.86)weeks in the study group were significantly different from those in the control group[(17.98±3.58)weeks, 20.71±2.46)weeks](t=4.026, 3.152, P<0.05). The VAS score(0.81±0.26)points and AO imaging score(0.52±0.13)points in the study group were lower than those in the control group [(1.26±0.58)points, 0.66±0.51)points], and the AOFAS score(89.51±9.85)points was higher than the control group(84.64±8.25)points(t=4.256, 4.726,2.423, P<0.05). There was no significant difference in the incidence of complications between the study group(4.54%) and the control group(9.09%)(χ2=0.469, P>0.05). Conclusion Posterior support plate fixation for the treatment of posterior tibiofibular fracture can improve ankle function, relieve pain, promote joint function recovery, shorten fracture healing time and complete weight-bearing time.
