控制性减压在治疗重型颅脑损伤患者中的效果分析

【摘要】 目的探导控制性减压在治疗重型颅脑损伤患者中的临床疗效。方法便利选取该院于2015年6月—2018年6月收治的100例患者为研究对象,随机分两组,治疗组50例(在重型颅脑损伤的患者中用控制性减压的方法治疗),对照组50例(在重型颅脑损伤的患者中用传统开颅快速减压的手术方法治疗)。比较两种手术方式的治疗效果。结果控制性减压治疗组与传统开颅对照组相比,术后并发症明显减少,术中急性脑膨出的概率4%、术后第1天迟发性颅内血肿的发生率2%均明显减少,两组比较,差异有统计学意义(χ~2=4.053、4.109,P<0.05)。术后1个月GCS评分明显升高,术后1个月死亡率明显下降,术后6个月GOS评分明显提高,差异有统计学意义(P<0.05)。结论控制性减压治疗重型颅脑损伤患者的疗效肯定,能减少脑再灌注损伤对脑组织的二次损害,降低脑急性膨出的概率,降低致死率及致残率,值得在基层医院推广使用。

【Abstract】 Objective To investigate the clinical efficacy of controlled decompression in the treatment of patients with severe craniocerebral injury. Methods One hundred patients admitted to our hospital from June 2015 to June 2018 were convenient randomly divided into two groups. The treatment group consisted of 50 patients(treated with controlled decompression in patients with severe head injury). 50 patients in the control group(treated in patients with severe craniocerebral injury with conventional craniotomy and rapid decompression). Compare the treatment effects of the two surgical methods.Results Compared with the traditional craniotomy group, the controlled decompression treatment group had significantly fewer postoperative complications. The probability of acute brain swelling during surgery was 4%, and the incidence of delayed intracranial hematoma on the first day after surgery was 2%. The difference was significantly reduced. The difference between the two groups was statistically significant(χ~2=4.053, 4.109, P<0.05). The GCS score was significantly increased in January, and the mortality was significantly decreased in the first month after operation. The GOS score was significantly improved at 6 months after operation, and the difference was statistically significant(P<0.05). Conclusion Controlled decompression is effective in treating patients with severe craniocerebral injury. It can reduce the secondary damage of brain reperfusion injury to brain tissue, reduce the probability of acute brain bulging, and reduce the mortality and disability rate.It is worthy of being promoted and used in the primary hospital.
