【摘要】 目的探讨前路钢板螺钉内固定治疗髋臼骨折伴骶髂关节脱位的疗效分析。方法方便选取2015年6月—2018年6月在该院骨科治疗的54例髋臼骨折伴骶髂关节脱位患者,随机分为两组,对照组采用双重建接骨板复位内固定术,观察组采用改良复位加压锁定接骨板内固定术,比较两组患者的各项手术指标、骨折疗效、骶髂关节间隙。结果观察组手术时间、术中出血量、术后VAS疼痛评分、住院时间均明显低于对照组(P<0.05);观察组Matta评分优良率92.59%明显高于对照组81.48%,差异有统计学意义(χ~2=4.477,P<0.05);观察组术后CT横断面及矢状面上骶髂关节间隙分别为(4.03±0.75)mm、(4.26±0.95)mm均明显小于对照组,差异有统计学意义(t=4.627、5.432,P<0.05)。结论前路钢板螺钉内固定治疗髋臼骨折伴骶髂关节脱位的疗效显著,复位固定效果好,创伤小,住院时间短,术后恢复快,具有积极的临床意义。更多还原

【Abstract】 Objective To investigate the effect of anterior plate and screw internal fixation in the treatment of acetabular fracture with sacroiliac joint dislocation. Methods Convenient select June 2015 to June 2018, a total of 54 patients with acetabular fracture and sacroiliac joint dislocation were randomly divided into two groups. The control group was treated with double reconstruction plate reduction and internal fixation, and the observation group was treated with modified reduction and compression locking plate fixation. The operative indexes, fracture effect and sacroiliac joint space of the two groups were compared. Results The operation time, intraoperative blood loss, postoperative VAS pain score, and hospital stay were significantly lower in the observation group than that in the control group(P<0.05). The excellent rate of the Matta score in the observation group was 92.59%, which was significantly higher than that in the control group(81.48%). And the difference between the two groups was statistically significant(χ~2=4.477, P<0.05). The CT cross-section and sagittal plane ankle joint space in the observation group were(4.03±0.75) mm and(4.26±0.95) mm, respectively, which were significantly smaller than in the control group. And the difference between the two groups was statistically significant(t =4.627, 5.432, P <0.05). Conclusion The treatment of acetabular fracture with sacroiliac joint dislocation by internal fixation with anterior plate and screw has positive clinical significance. It has remarkable effect, good reduction and fixation effect, little trauma,short hospital stay and the quick recovery after operation.更多还原
