无偿献血采血点初筛丙氨酸转氨酶升高的预防及纠正措施研究

【摘要】 目的探讨无偿献血采血点初筛丙氨酸转氨酶升高的预防及纠正措施。方法方便选取2017年1—12月在无偿献血采血点84例自愿无偿献血者为研究对象,将丙氨酸转氨酶水平>40 U/L定义为丙氨酸转氨酶水平过高,初筛不合格,将丙氨酸转氨酶水平≤40 U/L定义为丙氨酸转氨酶水平正常,初筛合格。对比初筛不合格组与初筛合格组的一般资料。结果该组84例无偿献血者中,丙氨酸转氨酶阳性13例,献血初筛检查不合格率为15.47%(13/84);另外71例丙氨酸转氨酶阴性,献血初筛检查合格率为84.52%(71/84)。初筛不合格组脂肪肝、长期熬夜、疲劳、饮酒、肥胖比例分别为2例(15.38%)、2例(15.38%)、3例(23.08%)、3例(23.08%)、3例(23.08%),明显高于初筛合格组4例(5.63%)、3例(4.22%)、6例(8.45%)、3例(4.22%)、0例(0.00%),差异有统计学意义(χ2=5.055 7、7.044 8、8.019 6、15.088 9、26.090 9,P<0.05)。结论丙氨酸转氨酶水平升高与脂肪肝、长期熬夜、疲劳、饮酒、肥胖有关,无偿献血工作中应加大宣传相关教育,提高献血有效率。

【Abstract】 Objective To investigate the prevention and corrective measures for the initial screening of alanine aminotransferase in unpaid blood donation sites. Methods From January to December 2017, 84 volunteers with voluntary blood donation at the blood donation site were convenient selected as subjects. The alanine aminotransferase level >40 U/L was defined as high alanine aminotransferase level, and the initial screening failed, the alanine aminotransferase level ≤ 40 U/L was defined as normal alanine aminotransferase level, the initial screening qualified. Compare the general information of the primary screening unqualified group and the primary screening qualified group. Results Among the 84 unpaid blood donors,13 were positive for alanine aminotransferase, and the unqualified rate of blood donation screening was 15.47%(13/84).Another 71 cases were negative for alanine aminotransferase of 84.52%(71/84). The proportions of fatty liver, long-term staying up late, fatigue, alcohol consumption and obesity in the unsuccessful screening group were 2 cases(15.38%), 2 cases(15.38%), 3 cases(23.08%), 3 cases(23.08%) and 3 cases(23.08%), which were significantly higher than the initial screening qualified group of 4 cases(5.63%), 3 cases(4.22%), 6 cases(8.45%), 3 cases(4.22%), and 0 cases(0.00%), the difference was statistically significant(χ~2=5.055 7, 7.044 8, 8.019 6, 15.088 9, 26.090 9, P<0.05). Conclusion The elevated alanine aminotransferase level is associated with fatty liver, long-term staying up late, fatigue, drinking, and obesity. In the work of voluntary blood donation, publicity and education should be increased to improve the efficiency of blood donation.
