血清尿素氮与肌酐对早期糖尿病肾病的诊断价值探讨

【摘要】 目的研究血清尿素氮(BUN)与肌酐(Scr)对早期糖尿病肾病的诊断中的价值。方法方便选取2015年1月—2018年12月该院就诊的糖尿病肾病患者500例,按尿微量蛋白排泄率分为两组作为观察组:临床糖尿病肾病组(A组UAER>300 mg/24 h)250例,早期糖尿病肾病组(B组UAER 30~300 mg/24 h)250例,以250名健康体检者(C组)作为对照组。结果观察A组、B组的血清尿素氮分别为(7.45±1.83)mmol/L、(5.51±1.17)mmol/L,肌酐分别为(113±17.26)μmol/L、(85±15.91)μmol/L,尿蛋白数值分别为(338±26.91)mg/24 h、(175±24.37)mg/24 h,均高于对照C组(4.48±1.45)mmol/L、(73±13.96)μmol/L、(21±31.82)mg/24 h。其中A、B组对比C组,血清尿素氮(t=14.3),肌酐(t=21.9);A、B组对比,血清尿素氮(t=10.4),肌酐(t=12.1),尿微量蛋白排泄率(t=19.6,P<0.05);B、C组对比,血清尿素氮(t=2.1),肌酐(t=1.9),(P>0.05);尿微量蛋白排泄率(t=10.7,P<0.05);A组、B组血清尿素氮、肌酐阳性检出率明显高于C组,血清尿素氮(χ~2=13.93),肌酐(χ~2=17.26),(P<0.05)。结论 BUN和Scr可有效防止DN的发生及发展,对于临床观察DN具有重要意义。

【Abstract】 Objective To study the value of serum urea nitrogen(BUN) and creatinine(Scr) in the diagnosis of early diabetic nephropathy. Methods A total of 500 patients with diabetic nephropathy who were admitted to our hospital from January2015 to December 2018 were convenient enrolled. The urinary microprotein excretion rate was divided into two groups as the observation group: clinical diabetic nephropathy group(UAER >300 mg/24 h in group A) 250 cases, in the early diabetic nephropathy group(group B UAER 30-300 mg/24 h) 250 cases, 250 healthy subjects(group C) as a control group. Results The serum urea nitrogen of group A and group B was(7.45±1.83) mmol/L,(5.51±1.17) mmol/L, and creatinine was(113±17.26) umol/L,(85±15.91) umol/L, urine protein values were(338±26.91)mg/24 h,(175±24.37)mg/24 h, respectively,which were higher than the control group C(4.48±1.45) mmol/L,(73±13.96)μmol/L,(21±31.82)mg/24 h. Among them, group A and group B were compared with group C, serum urea nitrogen(t=14.3), creatinine(t=21.9); group A, group B, serum urea nitrogen(t = 10.4), creatinine(t = 12.1), urine microprotein excretion rate(t=19.6, P<0.05); B, C group comparison,serum urea nitrogen(t=2.1),creatinine(t=1.9),(P>0.05); urine microprotein excretion rate(t=10.7, P<0.05), the positive rate of serum urea nitrogen and creatinine in group A and group B was significantly higher than that in group C, serum urea nitrogen(χ~2=13.93), creatinine(χ~2=17.26),(P<0.05). Conclusion BUN and Scr can effectively prevent the occurrence and development of DN, which is of great significance for clinical observation of DN.
