丹参酮ⅡA磺酸钠联合氯吡格雷对冠心病心绞痛疗效及血清NO、ET、TXB2水平变化

【摘要】 目的探讨冠心病心绞痛(AP)应用丹参酮ⅡA磺酸钠(STS)联合氯吡格雷治疗的临床效果及对患者血液血清一氧化氮(NO)、内皮素(ET)、血栓素B2(TXB2)水平的影响。方法方便选取该院2015年1月—2017年12月收治的90例冠心病AP患者,采用随机数字表法均分为两组。两组均接受基础治疗,对照组加用氯吡格雷治疗,观察组采用STS联合氯吡格雷治疗。观察两组治疗效果。结果相较于对照组,观察组治疗后AP发作次数更少[(0.57±0.29)次VS(0.91±0.37)次(t=6.658)]、平均持续时间更短[(3.72±0.94)minVS(7.07±1.22)min(t=7.662)]、平均所需硝酸甘油剂量更低[(0.34±0.13)mg/dVS(0.70±0.16)mg/d(t=5.487)](P<0.01);相较于对照组,观察组治疗后血清NO水平更高[(85.74±12.08)ng/LVS(69.42±10.30)ng/L(t=8.414)],ET[(37.46±7.03)μmol/LVS(46.32±7.43)μmol/L(t=6.174)]及TXB2T[(154.49±21.37)ng/LVS(192.82±24.76)ng/L(t=5.285)]水平更低(P<0.01);观察组治疗总有效率高于对照组(P<0.05);两组不良反应差异无统计学意义(P>0.05)。结论 STS联合氯吡格雷治疗冠心病AP疗效显著,可能与其有效提高血清NO水平,降低ET及TXB2水平有关。

【Abstract】 Objective To investigate the clinical effects of sodium tanshinone IIA sulfonate(STS) combined with clopidogrel in the treatment of coronary heart disease with angina pectoris(AP) and serum nitric oxide(NO), endothelin(ET) and thromboxane B2(TXB2) level of influence. Methods 90 patients with coronary heart disease admitted to our hospital from January 2015 to December 2017 were convenient selected and randomly divided into two groups. The two groups received basic treatment, the control group was treated with clopidogrel, and the observation group was treated with STS plus clopidogrel. The therapeutic effects of the two groups were observed. Results Compared with the control group, the number of AP episodes was lower in the observation group after treatment [(0.57±0.29) times VS(0.91±0.37) times(t=6.658)], and the average duration was shorter [(3.72±0.94) min VS(7.07±1.22) min(t=7.662)], the average required dose of nitroglycerin was lower [(0.34±0.13) mg/d VS(0.70±0.16) mg/d(t=5.487)](P<0.01); compared to the control group, serum NO levels were higher after treatment [(85.74±12.08) ng/L VS(69.42±10.30) ng/L(t=8.414)], ET[(37.46±7.03) μmol/L VS(46.32±7.43) μmol/L(t=6.174)] and TXB2 T [(154.49±21.37) ng/L VS(192.82±24.76) ng/L(t=5.285)] were lower(P<0.01); the total effective rate of the observation group was higher than that of the control group(P<0.05); there was no significant difference between the two groups(P>0.05). Conclusion The efficacy of STS combined with clopidogrel in the treatment of coronary heart disease is significant, which may be related to the effective increase of serum NO level and the decrease of ET and TXB2 levels.
