纳米碳在甲状腺癌中央区淋巴结清扫中的临床应用
Application of nano-carbon on central lymph node dissection of thyroid carcinoma
【摘要】 目的探讨纳米碳在甲状腺癌根治术中的作用。方法回顾性分析在我科行甲状腺全切除(甲状腺左叶、峡部及右叶全切除)加一侧或双侧VI区(即中央区)淋巴结清扫术并应用纳米碳的患者96例和行上述术式但不使用纳米碳的患者100例的临床资料。统计各组清扫的平均每侧的VI区淋巴结的数量、手术前后的血钙差值及手术时间。结果实验组(纳米碳组)清扫的平均每侧VI区的淋巴结数量为(5.8±1.7)枚,对照组(非纳米碳组)清扫的每侧VI区的淋巴结数量为(3.9±1.9)枚,两组差异具有统计学意义(P<0.05)。实验组行甲状腺全切除术+一侧或双侧VI区淋巴结清扫手术前后平均血钙差分别为(0.262±0.021)mmol/L、(0.307±0.026)mmol/L,而对照组分别为(0.558±0.032)mmol/L、(0.628±0.049)mmol/L,两组差异具有统计学意义(P<0.05)。实验组行甲状腺全切除术+一侧或双侧VI区淋巴结清扫的手术时间分别为(75±18)min、(84±20)min,而对照组为(96±21)min、(107±25)min,两组差异具有统计学意义(P<0.05)。结论应用纳米碳有助于确保VI区淋巴结清扫的彻底性、减少患者术后低钙血症的发生率、缩短手术时间。
【Abstract】 Objective To investigate the role of nano-carbon on opertation of radical thyroidectomy. Methods To retrospectively analyze clinical data of 96 patients(with nano-carbon) and 100 patients(without nano-carbon) who were undergone an operation of total thyroidectomy in addition one or both level VI lymph nodes dissection in our department. To respectively count the average number of per level VI lymph nodes, the difference value between preoperative and postoperative blood calcium and the surgery duration. Results The average number of per central lymph node in experimental group(with nano-carbon) was 5.8±1.7, the number of the control group was 3.9±1.9. The difference between the two groups was statistically significant(P<0.05). The mean difference value between preoperative and postoperative blood calcium with one or both level VI lymphadenectomy in experimental group were(0.262±0.021)mmol/L and(0.307±0.026)mmol/L; the D-values of the control group were(0.558±0.032)mmol/L and(0.628±0.049)mmol/L, respectively, the difference was statistically significant(P<0.05). The surgery duration of thyroidectomy in addition one or both central lymph nodes dissection in experimental group were(75±18)min and(84±20)min, respectively, the surgery duration of thyroidectomy in addition one or both central lymph nodes dissection in control group were(96±21)min and(107±25)min, respectively, the difference was statistically significant(P<0.05). Conclusion Nano-carbon contributes to ensure the thoroughness of central lymph node dissection, reduce the incidence of postoperative hypocalcemia and shorten the surgery duration.
