屈光参差性弱视儿童高阶像差的研究
Study on higher order aberrations in anisometropic amblyopia of children
【摘要】 目的研究屈光参差性弱视儿童高阶像差的特点,分析高阶像差在弱视形成过程中的可能作用机制,为弱视治疗提供依据。方法选取54例无斜视的屈光参差性弱视,年龄5~12岁,屈光度+3.0~+6.0 D,初诊患者,均为远视性屈光参差,分为正视眼组和弱视眼组,进行波前像差的检测;患者经过戴镜矫正、遮盖疗法、增视能训练系统、CAM训练、红光刺激等综合治疗2年后,按照矫正视力情况分为弱视治愈组、难治性弱视组和正视眼组,再次进行波前像差的检测。检验分析各组内差异。结果屈光参差性弱视患者正常眼组与屈光参差性弱视组比较,总像差及3、5阶像差在统计学上差异有显著性;治疗后,弱视治愈组及难治性弱视组和正视眼组组间比较在总像差、总高阶像差以及5阶像差存在着统计学差异(P<0.05)。结论高阶像差可能影响弱视患者的治疗效果,通过高阶像差的检测可能推测患者的大致预后。
【Abstract】 Objective To study the characteristics of higher order aberrations in anisometropic amblyopia of children, and analyze the possible mechanism of high order aberration in the forming of amblyopia,and to explore the practicability of aberrations correction therpy in amblyopia. Methods Fifty-four non-strabismus anisometropic amblyopia children aged from five to twelve years old, diopter from +3.0 D to +6.0 D, first visit, were selected. Then anisometropic amblyopia was divided into amblyopic eyes group and fellow eyes group. The wavefront aberrations were examined by the Allegretto wavefront analyzer. The amblyopia eyes group was given refractive errors correction, multi-media training system, covering treatment, CAM treatment and red light stimulation for 2 years. It was divided into recovered amblyopic group, intractable amblyopic and emmetropic group according to the best corrected visual acuity. All of the children were examined by the Allegretto wavefront analyzer once again. Interocular differences within groups were compared. Results Paired t-test showed significant differences in total aberration, third aberration and fifth aberration between anisometropic amblyopic eyes group and fellow eyes group. Posttreatment, there were significant differences in total aberration, total higher aberration and fifth aberration between recovered amblyopic group, intractable amblyopic and emmetropic group(P<0.05). Conclusion The higher order aberrations might affect the therapeutic effect of amblyopia. We might speculate the prognosis of amblyopia according to the high order aberration.
