不孕症患者甲状腺功能、雌激素水平、子宫内膜血流指标对FET结局的影响

【摘要】 目的 :探究不孕症患者甲状腺功能、雌激素水平、子宫内膜血流指标对冷冻胚胎移植技术(FET)结局的影响。方法 :整理我院诊治的不孕症患者94例,均行FET术(105周期),按照结局分为妊娠组(n=41)和未妊娠组(n=64)。比较两组患者的甲状腺激素、性激素水平及子宫内膜血流相关指标,并对上述指标与FET结局的关系进行Logistic回归分析。结果 :妊娠组患者血清游离甲状腺素(FT4)、雌二醇(E2)、子宫内膜血流指数(F1)均高于未妊娠组,促甲状腺激素(TSH)、孕酮(P)、孕酮/雌二醇(P/E2)水平均低于未妊娠组,且差异具有统计学意义。P/E2、TSH是FET结局的危险因素,其水平越高,未妊娠可能性越大;FT4、子宫内膜F1是保护因素,其水平越高,妊娠可能性越大。结论 :甲状腺功能、雌激素水平和子宫内膜F1均能影响FET结局。P/E2、TSH是FET结局的危险因素,FT4、子宫内膜F1是FET结局保护因素。

【Abstract】 Objective To investigate the effects of thyroid function, estrogen level and endometrial blood flow index on the outcome of frozen embryo transfer(FET) in infertile patients. Methods 94 infertility cases diagnosed and treated in our hospital were treated with FET(105 cycles) and divided into the pregnant group(n=41) and the non-pregnant group(n=64) according to the outcome. The levels of the thyroid hormone, sex hormone and endometrial blood flow related indexes in the two groups were compared, and the relationship between these indexes and FET outcome was analyzed using Logistic regression. Results The levels of the serum free thyroxine(FT4), estradiol(E2) and endometrial blood flow index(F1) in pregnant group were higher than those in non-pregnant group, and the levels of thyroid stimulating hormone(TSH), progesterone(P) and progesterone/estradiol(P/E2) were lower than those in non-pregnant group, with statistical difference. The levels of P/E2 and TSH are risk factors for FET outcome. The higher the levels of P/E2 and TSH, the greater possibility of non-pregnancy is. The levels of FT4 and endometrial F1 are protective factors. The higher the levels of P/E2 and TSH, the greater possibility of pregnancy is. Conclusion Thyroid function, estrogen level and endometrium F1 all affect FET outcome. The levels of P/E2 and TSH are risk factors for FET outcome, and the levels of FT4 and endometrial F1 are protective factors for FET outcome.
