液基薄层细胞学检查、人乳头瘤病毒DNA检测及阴道镜检查在宫颈癌筛查中的应用价值
Value of Thinprep Cytologic Test, Human Papillomavirus-DNA Combined with Colposcopy in Cervical Cancer Screening
【摘要】 目的探究液基薄层细胞学检查(TCT)、人乳头瘤状病毒(HPV)-DNA检测及阴道镜检查在宫颈癌筛查中的应用价值。方法选择2015年2月至2018年7月我院收治的宫颈异常患者纳入研究,所有患者均先接受TCT、HPV-DNA检查,后行阴道镜病理活检,以病理学诊断结果分析各诊断方法的诊断价值。结果 375例宫颈异常患者中,TCT阳性者49例,其中不典型鳞状细胞32例、鳞状上皮内低度病变11例、鳞状上皮内高度病变5例、鳞状上皮癌1例;HPV-DNA阳性者33例;阴道镜检查阳性者为46例;49例TCT阳性患者均经病理活检证实,其中CINⅠ级16例;CINⅡ级10例,CINⅢ级3例,浸润癌1例;三者联合诊断CINⅠ级、CINⅡ级患者准确性明显高于TCT联合HPV-DNA检查,差异有统计学意义(P<0.05)。结论 TCT、HPV-DNA及阴道镜三者联合可提高宫颈癌筛查的诊断效能。
【Abstract】 Objective To investigate the application value of thinprep cytologic test(TCT), human papillomavirus(HPV)-DNA test and colposcopy in the cervical cancer screening.Methods All the patients with cervical abnormalities admitted to our hospital from February 2015 to July 2018 were enrolled in the study. All the patients underwent TCT and HPV-DNA tests, and then colposcopy biopsy. The diagnostic value of each diagnostic method was analyzed by pathological diagnosis.Results Of the 375 patients, 49 patients were positive for TCT, including 32 patients with atypical squamous cell, 11 patients with low-grade squamous intraepithelial lesion, and 5 patients with high-grade squamous intraepithelial lesion, 1 patient with squamous cell cancer. 33 patients with positive HPV-DNA were observed; 46 patients with positive colposcopy were observed; 49 patients with TCT biopsy positive were confirmed by pathological biopsy, including 16 patients with CINⅠ grade; 10 patients with CINⅡ, 3 patients with CINⅢ grade, 1 patient with invasive cancer. The accuracy of the combined diagnosis of patients with CINⅠ grade, CINⅡ grade was significantly higher than those of TCT combined with HPV-DNA, and the difference was statistically significant(P<0.05).Conclusion The combination of TCT, HPV-DNA and colposcopy can improve the diagnostic efficacy of cervical cancer screening.
