CBCT对肺癌SBRT摆位误差及靶区外放边界的校正价值

【摘要】 目的通过锥形束CT(CBCT)图像配准确定早期非小细胞肺癌体部立体定向放射治疗(SBRT)的摆位误差探讨MPTV靶区勾画外扩范围,为临床提供参考数据。方法选取13例早期非小细胞肺癌患者,应用CBCT对患者进行治疗前扫描,并记录摆位误差。根据MPTV外扩公式(MPTV=2.5∑+0.7σ)得出靶区(PTV)的摆位误差范围。结果 X平移=(0.16±0.14)cm、Y平移=(0.45±0.37)cm、Z平移=(0.21±0.21)cm; X旋转=(0.45±0.52)°、Y旋转=(1.26±1.39)°、Z旋转=(0.84±0.82)°。MPTV在X轴、Y轴、Z轴的外扩范围分别为0.51 cm、1.01 cm、0.58 cm。结论采用CBCT图像引导患者靶区位置校准,可准确得到非小细胞肺癌患者因摆位误差导致的PTV外扩边界,为临床靶区勾画范围提供参考依据。

【Abstract】 Objective To investigate the setup error and the determination of target position based on cone beam CT for early non-small cell lung cancer(NSCLC) by stereotactic body radiation therapy,and to explore the target margin of MPTV so as to provide reference data for clinical practice.Methods A total of 13 patients with early NSCLC were enrolled in the study. Before treatment, the patients underwent CBCT,and the setup error was recorded.According to the formula(MPTV=2.5∑+0.7σ), and the setup error range of PTV was got.Results The translational setup errors on X axis(left-right,LR),Y axis(superior-inferior,SI) and Z axis(anterior-posterior,AP) were(0.16±0.14)cm,(0.45±0.37)cm,(0.21±0.21)cm,respectively,and the rotational setup errors on X axis,Y axis,Z axis were(0.45±0.52)°,(1.26±1.39)°,(0.84±0.82)°,respectively. According to the formula,and the translational ranges of MPTV on X axis,Y axis,Z axis were 0.51 cm,1.01 cm,0.58 cm,respectively.Conclusion The setup error coming from CBCT images can calculate the margin of PTV in patients with NSCLC accurately,which can provide reference data for clinical practice.
