血清miR-155、HIF1α的表达与急性心肌梗死患者预后的关系

【摘要】 目的探讨急性心肌梗死患者血清miR-155和缺氧诱导因子1a(HIF1α)表达水平及其与患者预后的关系。方法将2016年1月至2017年7月收治的100例急性心肌梗死患者作为心梗组,同期100例健康者为对照组,心梗组入院后12 h内行急诊经皮冠状动脉介入治疗(PCI),并随访1年,分析患者入院时血清miR-155、HIF1α水平与PCI术后左心功能变化及心血管事件发生的关系。结果心梗组PCI术前后血清miR-155表达水平均高于对照组(P<0.05),HIF1α水平低于对照组(P<0.05);心梗组血清miR-155、HIF1α水平之间呈负相关关系(r=-0.729,P=0.001);不同miR-155、HIF1α水平患者术后半年LVEF、LVEDV、LVESV指标比较差异有统计学意义(P<0.05);miR-155高表达组PCI术后1年心血管事件发生率高于miR-155低表达组(P<0.05),HIF1α高表达组PCI术后1年心血管事件发生率低于HIF1α低表达组(P<0.05)。结论血清miR-155、HIF1α表达水平对评估急性心肌梗死患者PCI术后左心功能和心血管事件发生率具有重要价值。

【Abstract】 Objective To investigate the expressions of serum miR-155 and hypoxia inducible factor 1 a(HIF1α) in patients with acute myocardial infarction(AMI) and their relationships with patient’s prognosis.Methods One hundred patients with acute myocardial infarction who were admitted and treated in our hospital from January 2016 to July 2017 were enrolled as AMI group,at the same time,100 healthy subjects were enrolled as control group.The patients in AMI group were treated by emergency percutaneous coronary intervention(PCI) within 12 h after admission,with 1-year follow up.The relationships between the serum levels of miR-155,HIF1α and left ventricular function as well as cardiovascular events after PCI was analyzed.Results The expression levels of serum miR-155 in AMI group before and after PCI were significantly higher than those in control group(P<0.05),however,the levels of HIF1α were significantly lower than those in control group(P<0.05).There was a negative correlation between miR-155 levels and HIF1α levels in AMI group(r=0.729,P<0.01).There were significant differences in LVEF,LVEDV and LVESV in the patients with different levels of miR-155 and HIF1α in half a year after operation(P<0.05).Moreover the incidence rate of cardiovascular events within 1 year after PCI in miR-155 high expression group was significantly higher than that in miR-155 low expression group(P<0.05),however,the incidence rate of cardiovascular events within 1 year after PCI in HIF1α high expression group was significantly lower than that in HIF1α low expression group(P<0.05).Conclusion The expression levels of serum miR-155 and HIF1α have important significance in evaluating left ventricular function and incidence of cardiovascular events in patients with AMI after PCI.
