【摘要】 目的:探讨超声引导下竖脊肌平面阻滞在乳腺癌术后镇痛中的应用效果。方法:选取2018年6月至2019年6月江门市中心医院收治的乳腺癌患者134例为研究对象,按随机数表法分为对照组和观察组,每组67例。对照组采用自控镇痛泵,观察组在对照组基础上行超声引导下竖脊肌平面阻滞。比较两组患者术后1、6、12、24 h Prince-Henry疼痛评分(PHPS)、按压镇痛泵次数、术中麻醉药用量及术后不良反应发生情况。结果:观察组术后1、6、12、24 h PHPS评分均低于对照组(P<0.05)。观察组按压镇痛泵次数、术中丙泊酚和瑞芬太尼用量均少于对照组(P<0.05)。观察组不良反应发生率低于对照组(P<0.05)。结论:超声引导下竖脊肌平面阻滞术可有效缓解乳腺癌患者术后疼痛,减少术中、术后镇痛药物使用量,降低不良反应发生率。更多还原

【Abstract】 Objective:To study the efficacy of ultrasound-guided erector spinae plane block in postoperative analgesia among patients with breast cancer.Methods:Included in this study were 134 patients with breast cancer who were diagnosed and treated in Jiangmen Central Hospital between June 2018 and June 2019.According to random number table,the subjects were randomly divided into the study group(n= 67)and the control group(n= 67).The control group were given patient-controlled analgesia while the study group were additionally given ultrasound-guided erector spinae plane block.The Prince-Henry pain scores(PHPS)at 1 h,6 h,12 h and 24 h after surgery and the frequency of analgesic pump-pressing,intraoperative use of analgesic drugs,incidence of postoperative adverse reactions were compared between the two groups.Results:The PHPS scores in the study group at 1 h,6 h,12 h and 24 h after surgery were significantly lower than those in the control group(allP<0.05).The frequency of analgesic pump-pressing,intraoperative use of propofol and remifentanil in the study group were significantly less than those in the control group(allP<0.05).The incidence of adverse reactions in the study group was significantly lower than that in the control group( P< 0.05).Conclusion:Ultrasound-guided erector spinae plane block may effectively relieve postoperative pain,lessen the need for analgesic drugs during and after surgery,and reduce the incidence of adverse reactions among patients with breast cancer.更多还原
