拔牙窝内即刻植入Bio-Oss颗粒联合Bio-Gide胶原膜表面覆盖在磨牙区拔牙中的应用

【摘要】 目的:探究拔牙窝内即刻植入Bio-Oss颗粒联合Bio-Gide胶原膜表面覆盖在磨牙区拔牙中的应用效果。方法:选取2016年1月~2018年6月我院60例磨牙区拔牙患者,依据随机数字表法分组,各30例。两组均行磨牙区拔牙,对照组拔牙后行常规处理,观察组拔牙后即刻于拔牙窝内植入Bio-Oss颗粒,并于表面覆盖Bio-Gide胶原膜。统计对比两组临床效果、新骨轮廓及骨密度评分,并对比拔牙前、拔牙6个月后牙槽骨宽度与高度。结果:两组拔牙后均无并发症发生,拔牙伤口愈合良好。1个月,患者牙龈基本愈合,拔牙伤口关闭; 3~6个月,对照组颊舌侧牙龈愈合,但牙槽骨外形较拔牙前缩窄,牙槽骨宽、高度则减少,特别是颊侧凹陷显著;观察组胶原膜被牙龈覆盖,颊舌侧牙龈呈平台状愈合,牙槽骨外形丰满;观察组新骨轮廓、骨密度评分较对照组高(P<0.05);两组拔牙后6个月牙槽骨宽度、高度均较拔牙前有所下降,但观察组高于对照组(P<0.05)。结论:磨牙区拔牙患者拔牙后于拔牙窝内即刻植入Bio-Oss颗粒联合Bio-Gide胶原膜表面覆盖,可显著减少因拔牙造成的牙槽骨吸收,维持拔牙位点骨量,为后期种植牙修复提供良好位点条件,且安全性高。

【Abstract】 Objective: To investigate the practical efficacy of immediate implantation of Bio-Oss particles covered with Bio-Gide collagen membrane into extraction sockets in the molar region. Methods: Included in this study were 60 cases of molar teeth extraction in our hospital between January 2016 and June 2018,who were assigned to two groups according to random number table( n = 30 each). The both groups underwent tooth extraction in the molar region. After the tooth extraction,the control group received routine treatment,while the study group received immediate implantation of Bio-Oss particles covered with Bio-Gide collagen membrane into the extraction sockets. The clinical efficacy,bone contour and bone density scores in the two groups were compared,and the alveolar bone width and height were also compared between the baseline and after tooth extraction. Results: The both groups experienced good healing of the extraction wounds without any complications. The gingiva became basically healed and the tooth extraction wounds closed at one month later.After 3 to 6 months,the control group showed complete healing of the buccolingual gingiva,yet the narrower alveolar bone contour,reduced width and height of the alveolar bone led to significant depression at the buccal side. In the study group,the collagen membrane had been embedded by gingiva,the buccolingual gingiva showed healing alongside a platform,the alveolar bone appeared fully grown,and the new bone contour and bone density score were higher than those in the control group( P< 0.05). The width and height of alveolar bone in the two groups at 6 months after the tooth extraction were lowered than baseline,however,these data were still greater in the study group than in the control group( P < 0. 05). Conclusion After tooth extraction in the molar region,immediate implantation of Bio-Oss particles covered with Bio-Gide collagen membrane into the extraction socket may significantly reduce alveolar bone resorption caused by the tooth extraction,preserve the bone mass at the extraction site,and offer favorable site conditions for future dental restoration,meanwhile with reliable safety.
