【摘要】 目的观察Quadrant通道系统下手术治疗单节段腰椎椎间盘突出症的临床疗效。方法将60例单节段腰椎椎间盘突出症患者,随机分为观察组与对照组,各30例。观察组患者在Quadrant通道系统下手术治疗,对照组患者采用传统开放性手术治疗。比较两组患者手术时间、切口长度、术后并发症发生率,以及两组患者术前及术后1个月、6个月、12个月的疼痛视觉模拟量表(VAS)评分、腰椎Oswestry功能障碍指数(ODI);术后12个月采用改良的MacNab标准评价两组患者临床疗效。结果两组患者手术时间、切口长度、并发症发生率及优良率比较,差异均无统计学意义(均P>0. 05)。观察组术后各时间点的VAS评分及ODI均低于对照组(均P <0. 05)。结论相对于传统开放手术,Quadrant通道系统下手术治疗单节段腰椎椎间盘突出症,更有利于缓解患者疼痛症状及改善腰椎功能,临床效果更好。更多还原

【Abstract】 Objective To observe the clinical efficacy of surgical treatment under Quadrant channel system for single-segment lumbar intervertebral disc herniation. Methods Sixty patients with single-level lumbar disc herniation were randomly divided into observation group and control group,with 30 cases in each group. Patients in the observation group received surgical treatment under Quadrant channel system,while those in the control group were treated with traditional open surgery. Operation duration,incision length,and incidence rates of postoperative complications,as well as the Visual Analogue Scale( VAS) pain scores and Oswestry disability index( ODI) of lumbar spine before and 1,6 and 12 months after operation were compared between the two groups; twelve months after operation,the clinical efficacy of patients in the two groups was evaluated by modified MacNab criteria.Results There were no statistically significant differences in operation duration incision length,incidence rates of complications,and excellence and good rate between the two groups( all P > 0. 05). The observation group had lower VAS scores and ODI than the control group at all time points after operation( all P < 0. 05).Conclusion Compared with traditional open surgery,surgical procedure under Quadrant channel system has a lower incidence rate of postoperative complications in the treatment of single-segment lumbar disc herniation,and is more conducive to relieving patients’ pain symptoms and improving lumbar function,achieving a favorable clinical effect.更多还原
