高频重复经颅磁刺激结合认知康复训练在血管性认知障碍患者中的临床应用分析
Analysis on clinical application of high frequency repetitive transcranial magnetic stimulation combined with cognitive rehabilitation training in patients with vascular cognitive impairment
【摘要】 目的探讨分析高频重复经颅磁刺激结合认知康复训练在血管性认知障碍患者中的临床应用。方法选取我院收治的血管性认知障碍患者50例,将患者分为研究组和对照组,给予两组患者常规药物:控制血压、血糖、调脂、改善微循环,营养脑神经等治疗;给予对照组患者认知康复训练,给予研究组患者认知康复训练联合高频重复经颅磁刺激干预,对两组患者认知功能,ADL能力改善情况进行比较分析。结果给予两组患者不同干预模式之后,研究组患者MMSE评分(23.9±3.42)分、MoCA评分(21.4±2.13)分,改良Barthel指数(MBI)评分(评估ADL)(69.2±4.1)分,改善情况显著优于对照组(19.5±2.61)分,(19.12±2.48)分,(61.7±4.3)分,差异有统计学意义(P <0.05);两组患者在MoCA评分中的注意力、视空间与执行能力、延迟回忆等方面差异有统计学意义(P <0.05)。结论给予血管性认知障碍患者认知康复训练可以有效改善其认知功能及提升日常生活能力;给予血管性认知障碍患者高频重复经颅磁刺激结合认知康复训练干预模式,能够更显著改善患者认知能力,提升日常生活能力。
【Abstract】 Objective To explore and analyze the clinical application of high frequency repetitive transcranial magnetic stimulation combined with cognitive rehabilitation training in patients with vascular cognitive impairment.Methods 50 patients with vascular cognitive impairment who were admitted and treated in our hospital were selected and they were given routine drugs to control blood pressure and blood sugar, regulate lipid, improve microcirculation and brain nerve nutrition. Patients were divided into the study group and the control group. Patients in the control group were given cognitive rehabilitation training while the study group was given cognitive rehabilitation training combined with high frequency repetitive transcranial magnetic stimulation. The cognitive function and improvement of ADL ability of patients in two groups were compared and analyzed. Results After giving different intervention modes to patients of two groups, MMSE score(23.9±3.42), MoCA score(21.4±2.13), modified Barthel index(MBI) score(69.2±4.1) in the study group were significantly better than those in the control group(19.5±2.61),(19.12±2.48)and(61.7±4.3). The difference was significant(P < 0.05). There were significant differences in attention, visual space, executive ability and delayed recall in MoCA score between the two groups(P < 0.05). Conclusion Cognitive rehabilitation training for patients with vascular cognitive impairment can effectively improve their cognitive function and enhance their activity of daily living ability. High frequency repetitive transcranial magnetic stimulation combined with cognitive rehabilitation training intervention mode for patients with vascular cognitive impairment can significantly improve cognitive ability and enhance activity of daily living ability.
