鼻塞持续气道正压辅助通气治疗新生儿重症湿肺的效果及对肺功能和血清S100A8、S100A9蛋白水平的影响

【摘要】 目的观察鼻塞持续气道正压辅助通气治疗新生儿重症湿肺的效果及对肺功能和血清S100钙结合蛋白A8(S100A8)、S100钙结合蛋白A9(S100A9)水平的影响。方法选取我院2015年5月~2018年2月收治的80例新生儿重症湿肺患儿为对象,采用随机数字表法将其分为对照组和实验组各40例,对照组采用常规治疗,实验组采用鼻塞持续气道正压辅助通气治疗。比较两组的临床疗效、肺功能[(FEVl)、(FEVl/FVC)、PEF)]及血清S100A8、S100A9蛋白水平变化。结果治疗前两组患儿的FEVl、FEVl/FVC、PEF值和血清S100A8、S100A9蛋白水平相较,差异无统计学意义(P> 0.05),治疗后实验组的FEVl、FEVl/FVC、PEF均高于对照组,血清S100A8、S100A9蛋白水平均低于对照组,差异有统计学意义(P <0.05);实验组总有效率为87.50%,明显高于对照组的70.00%,差异有统计学意义(P <0.05)。结论鼻塞持续气道正压辅助通气治疗新生儿重症湿肺的疗效显著,能够有效的缓解肺功能,降低血清S100A8、S100A9蛋白水平,值得进一步推广使用。
【Abstract】 Objective To the observe the effect of continuous positive airway pressure assisted ventilation on neonatal severe wet lungs and its influence on lung function and serum levels of S100 calcium binding protein A8(S100 A8)and S100 calcium binding protein A9(S100 A9). Methods Eighty neonates with severe wet lung treated in our hospital from May 2015 to February 2018 were randomly divided into experimental group and control group,with40 cases in each group.The control group received regular treatment,while the experimental group was treated with nasal obstruction sustained positive airway pressure assisted ventilation.The clinical effects,pulmonary function[(FEVl),(FEVl/FVC)、(PEF)]and protein changes of serum S100 A8 and S100 A9 between the two groups were compared.Results Before treatment,there were no significant differences in the FEVl,FEVl/FVC,PEF,and protein levels of serum S100 A8 and S100 A9 between the two groups(P > 0.05).After treatment,the FEVl,FEVl/FVC and PEF levels in the experimental group were all higher than those in the control group,the protein levels of serum S100 A8 and S100 A9 in the experimental group were all lower than those in the control group,and the differences were statistically significant(P < 0.05).The total effective rate in the experimental group was 87.50%,which was significantly higher than that in the control group of 70%,and the difference was statistically significant(P < 0.05). Conclusion Nasal continuous positive airway pressure assisted ventilation has significant effect on neonatal severe wet lung,can effectively alleviate the pulmonary function and reduce the protein levels of serum S100 A8 and S100 A9,which is worthy of further application.
