基于MSCT的肩关节骨性结构与肩袖损伤的关系探讨

【摘要】 目的基于MSCT技术探讨肩关节骨性结构与肩袖损伤的关系。方法选择2017年1月~2018年5月在我院确诊为肩袖损伤的患者50例为观察组,另选择健康志愿者100例为对照组。采用64层螺旋CT进行肩关节扫描,观察指标包括肱骨头侧VA、IA、CROC、AROC、HTRA、THCD,肩胛盂侧VA、IA、CROC、AROC、CVCA、AVCA,头盂匹配指标:CCFI、ACFI、CSI、HGSO、HGMO和其他指标:HAD、TAD、ACD。结果两组头侧相关指标比较:观察组HTRA显著大于对照组,差异有统计学意义(P <0.05)。两组盂侧各指标比较:观察组VA、IA、AVCA显著大于对照组,差异有统计学意义(P <0.05)。两组其他指标比较:观察组HAD显著低于对照组,差异有统计学意义(P <0.05)。结论肩袖损伤患者肩关节骨性结构头结节仰角偏大、肩胛盂侧倾斜角偏大,轴位曲度半径较短,冠状位嵌合指数偏大,头顶肩峰距偏小,提示这些骨性指标可能与肩袖损伤有关。

【Abstract】 Objective To discuss relationship between bony structures of shoulder joints and rotator cuff injury based on MSCT. Methods 50 patientswho were diagnosed with rotator cuff injury in our hospitalfrom January 2017 to May 2018 were selected as the observation group,and 100 healthy volunteers from January 2017 to May 2018 were selected as control group.16-slice spiral CT was used to scan the shoulder joint.Observation indexes included VA,IA,CROC,AROC,HTRA and THCD in the side of humeral head,VA,IA,CROC,AROC,CVCA and AVCA in the side of glenoid cavity,cephalo-pelvis matching indexes such as CCFI,ACFI,CSI,HGSO,and HGMO and other indexes such as HAD,TAD and ACD. Results The comparison of related indexes of humeral head showed that HTRA in the observation group was significantlybigger than that in the control group.The difference was statistically significant(P< 0.05).Comparison of various indexes of the pelvis side between the two groups showed that VA,IA and AVCA in the observation group were significantly bigger than those in the control group.The difference was statistically significant(P< 0.05).Comparison ofother indexesbetween the two groups showed that HAD in the observation group was lower than that in the control group.The difference was statistically significant(P < 0.05). Conclusion In patients with rotator cuff injury, the elevation angle of head nodules of bony structure of shoulder joint is relatively large.The oblique angle of glenoid cavity is relativelylarger,the radius of axial position is relativelyshort,the chimericindex of coronary position is relatively large and the acromion distance overheadis relativelysmall.These bone indicators may be related to rotator cuff injury.
