超声引导下脑卒中后偏瘫肩痛的临床特征及疗效研究
Clinical characteristics and therapeutic study of ultrasound-guided in shoulder pain of hemiplegia after stroke
【摘要】 目的:利用肌骨超声诊断技术探索脑卒中后偏瘫肩痛的病因组成特点,并探讨超声引导下注射治疗对脑卒中偏瘫肩痛的康复治疗效果。方法:选取2018年7月至2019年6月广西壮族自治区人民医院脑卒中后偏瘫肩痛患者50例,利用肌骨超声技术进行诊断,分析脑卒中偏瘫肩痛的病因及临床特点,选择其中30例作为观察组,行超声引导下靶向药物注射治疗。另选取30例脑卒中偏瘫肩痛并接受常规康复诊疗患者30例作为对照组。比较两组患者的疼痛程度变化、肩关节活动度、运动功能评定及日常生活能力(ADL)。结果:50例脑卒中后肩痛患者中,肱二头肌长头肌腱炎症(50%)及肩峰—三角肌下滑囊炎症(48%)占比最高。与对照组比较,经超声引导下注射治疗后,观察组视觉模拟评分(VAS)、简式Fugl-Meyer运动功能评分(FMA-U)、改良Barthel指数(MBI)及肩关节活动度均明显改善(P<0. 05)。结论:利用肌骨超声技术诊断及治疗可显著提高脑卒中后肩痛的诊断率,超声引导下康复治疗可有效缓解患者肩部疼痛,提高偏瘫上肢功能,改善ADL。
【Abstract】 Objective: To explore the clinical characteristics of shoulder pain after stroke by using musculoskeletal ultrasound diagnosis technology,and to explore the effect of ultrasound-guided injection therapy. Methods:A total of 50 patients with shoulder pain after stroke were diagnosed by musculoskeletal ultrasound technique.Among them,30 patients with injection therapy indications were selected as an observation group. 30 patients with hemiplegia and shoulder pain who received routine rehabilitation treatment were defined as a control group. The pain degree,shoulder mobility,motor function assessment,and the activity of daily living(ADL)were observed and compared. Results: In 50 patients with shoulder pain,the proportion of biceps brachii tarsal inflammation(50%)was the highest,followed by acromion-delta glial inflammation(48%). Compared with the control group,the visual simulation score(VAS),Fugl-Meyer motor function score(FMA-U),improved Barthel index(MBI)and shoulder joint mobility were improved significantly in the observation group after ultrasound-guided injection(P<0. 05). Conclusion: Musculoskeletal ultrasound-guided diagnosis and treatment can significantly elevate the diagnostic rate of patients with shoulder pain after stroke. Ultrasound-guided injection can alleviate the shoulder pain,and improve upper extremity function of hemiplegia and ADL.
