【摘要】 目的研究超声介入治疗肩峰下撞击综合征(SIS)患者的疗效及复发危险因素。方法回顾性分析92例SIS患者的临床资料,根据其超声表现进行分型,并分别采用超声引导肩峰下滑囊注射(单纯注射组)以及超声引导肩袖肌腱松解术联合肩峰下滑囊注射(联合治疗组),比较两组术后1周及1,3,6月的疗效差别。结果两组患者术后1周及1,3,6月的VAS,CMS和OSS评分较术前均有不同程度改善,但术后6月均有复发病例,单纯注射组及联合治疗组的晚期复发率分别为34.9%及10.2%,两组差别有统计学意义(P<0.05)。结论对于SIS的患者,采用超声引导下介入治疗有确切的临床疗效;但对于超声诊断为SIS 2型的患者,应采用超声引导肩袖肌腱松解术联合肩峰下滑囊注射,以降低晚期复发风险。更多还原

【Abstract】 Objective To investigate the efficacy and the relapsing risks after ultrasound-guided interventional therapies for subacromial impingement syndrome(SIS). Methods A retrospective clinical study was conducted. 92 patients with SIS were divided into two groups according to different ultrasound-imaged types:43 of whom received ultrasound-guided subacromial-subdeltoid(SASD)bursa injection(group 1)while 49 received ultrasound-guided rotator cuff fenestration with SASD injection(group 2). Outcomes were compared at 1 week and 1,3 and 6 months post-therapies. Results The VAS,CMS and OSS scores of the two groups all improved after therapies. However the study also revealed relapse in the late stage. The mean relapsing risk in group 1 was 34.9% vs 10.2%in group 2(P<0.05). Conclusions Patients with SIS were generally functioned well after ultrasound-guided interventional therapies. However,the patients with Type 2 SIS will receive a great reduction of relapsing risk through ultrasound-guided rotator cuff fenestration with SASD injection.更多还原
