纤维蛋白-1与2型糖尿病大血管病变的相关性分析

【摘要】 目的观察血浆纤维蛋白-1(Fibulin-1)水平与2型糖尿病(T2DM)患者发生大血管病变的相关性。方法随机选取T2DM患者56例,其中单纯T2DM患者(T2DM组)28例,合并大血管病变的T2DM患者(MVC组)28例,收集同时期无糖尿病但有显著心脑血管疾病患者(CCD组)28例及健康体检者(NC组)28例。收集全部研究对象的临床资料,主要包括身高(H)、体质量(W)、计算出体质量指数(BMI),询问病史,测定空腹血糖(FPG)、甘油三酯(TG)、胆固醇(TC)等指标;免疫比浊法检测糖化血红蛋白(HbA1c)水平,采用酶联免疫法(ELISA法)测定血浆Fibulin-1水平,收集记录相关数据并进行统计分析。结果 MVC、T2DM组FPG、BMI水平高于CCD、NC组(P<0.05);MVC、T2DM、CCD组Fibulin-1水平高于NC组(P<0.05),且MVC组Fibulin-1水平高于T2DM、CCD组(P<0.05);MVC组HbA1c水平高于T2DM、CCD、NC组(P<0.05),T2DM组HbA1c水平高于CCD、NC组(P<0.05);Fibulin-1与FPG、HbA1c、BMI、年龄呈正相关(P<0.05),而与TC、TG无相关性(P>0.05);年龄、HbA1c是Fibulin-1的独立危险因素。结论 Fibulin-1水平与T2DM发病有相关,有可能是独立于血脂外,与血糖呈正相关性改变;同时可能参与了T2DM并发大血管病变的发生和发展过程。

【Abstract】 Objective To observe the correlation between plasma Fibulin-1 level and macrovascular disease in patients with type 2 diabetes mellitus(T2DM).Methods 56 patients with T2DM were randomly selected,including 28 patients with T2DM(T2DM group) and 28 patients with T2DM with macrovascular disease(MVC group).28 patients with no diabetes mellitus but significant cardiovascular and cerebrovascular disease(CCD group) and 28 healthy subjects(NC group) were collected as control group.The clinical data of all subjects were collected,including height(H),body mass(W),calculate body mass index(BMI) and medical history.The fasting blood glucose(FPG),triglyceride(TG),cholesterol(TC) and other indicators were measured and the level of glycated hemoglobin(HbA1c) was detected by immunoturbidimetric assay.Plasma Fibulin-1 levels were determined by enzyme-linked immunosorbent assay(ELISA),and relevant data were collected and statistically analyzed.Results The levels of FPG and BMI in MVC and T2DM group were higher than those in CCD and NC group(P<0.05).The levels of Fibulin-1 in MVC,T2DM and CCD group were higher than those in NC group(P<0.05),and the levels of Fibulin-1 in MVC group were higher than those in T2DM and CCD group(P<0.05).The levels of HbA1c in MVC group were higher than those in T2DM,CCD and NC group(P<0.05) and the levels of HbA1c in T2DM group were higher than those in CCD and NC group(P<0.05).The Fibulin-1 was positively correlated with FPG,HbA1c,BMI and age(P<0.05),but not with TC and TG(P>0.05).Age and HbA1c were independent risk factors for Fibulin-1.Conclusion Fibulin-1 level is associated with T2DM.It may be independent of blood lipids and positively correlated with blood glucose and participate in the occurrence and development of T2DM complicated with macrovascular disease.
