Klotho及TGF-β1在慢性肾脏病患者中的检测及临床意义
Detection and Clinical Cignificance of Klotho and TGF-β1 in Patients with Chronic Kidney Disease
【摘要】 目的测定血Klotho、尿TGF-β1在慢性肾脏病(CDK)患者不同肾功能状态下水平变化,研究Klotho及TGF-β1对慢性肾脏病检测的临床意义。方法选取佳木斯大学附属第一医院慢性肾脏病患者142例作为实验组,将其分为5组:CKD1期、CKD2期、CKD3期、CKD4期、CKD5期,分别为25例、28例、32例、30例、27例。对照组选取同时期来本院门诊健康体检患者26例。抽取该实验对象血清样本,通过ELISA法检测血Klotho、尿TGF-β1浓度,进一步分析这两种蛋白与慢性肾脏病的关系。结果血清Klotho蛋白随着肾脏病的进展逐渐降低(P<0. 01),且健康对照组水平逐渐明显降低,具有明显差异性(P<0. 01);尿TGF-β1随着肾脏病的进展逐渐升高且与血尿素氮及肌酐呈正比,但在CKD4期及CKD5期反而逐渐降低并且与肌酐尿素氮升高呈负相关(P<0.01)。结论随着慢性肾脏病的进展,血Klotoh蛋白逐渐降低,尿TGF-β1在慢性肾脏病1期,2期,3期中逐渐升高,但在慢性肾脏病4期、5期中反而逐渐降低,因此联合检测患者血清Klotho蛋白、尿TGF-β1的浓度可作为评估慢性肾脏病的严重程度及预后的重要指标,更好的为临床医生诊断及治疗该疾病。
【Abstract】 Objective To determine the changes of blood Klotho and urinary TGF-β1 levels in patients with chronic kidney disease( CDK) under different renal functions. Klotho and T were used to detect chronic kidney disease and its clinical significance.Methods 142 patients with chronic kidney disease affiliated to the First Affiliated Hospital of Jiamusi University were selected as the experimental group.They were divided into 5 groups: CKD1( 25 cases),CKD2( 28 cases),CKD3( 32 cases),CKD4( 30 cases),and CKD5( 27 cases). In the control group,26 patients from the outpatient clinic were enrolled in the same period.The serum samples of the experimental subjects were taken,and the blood Klotho and urinary TGF-β1 concentrations were measured by ELISA,and the relationship between the two proteins and chronic kidney disease was further analyzed. Result Serum Klotho protein decreased gradually with the progression of kidney disease( P<0.01),and the level of healthy control group gradually decreased,with significant difference( P<0.01).Urine TGF-β1 with the progress of the kidney disease gradually increased and was proportional to blood urea nitrogen and creatinine,but decreased gradually in CKD4 and CKD5 and negatively correlated with creatinine urea nitrogen. The difference was statistically significant( P < 0.01).Conclusion With the progress of chronic kidney disease,blood Klotoh protein gradually decreased,and urinary TGF-β1 gradually increased in stage 1,2,and stage 3 of chronic kidney disease,but gradually decreased in stage 4 and stage 5 of chronic kidney disease.Therefore,the combined detection of serum Klotho protein and urinary TGF-β1 concentration can be used as an important indicator to assess the severity and prognosis of chronic kidney disease,and better for the diagnosis and treatment of the disease by clinicians.
