地塞米松、脑苷肌肽联合高压氧舱治疗一氧化碳中毒的早期临床疗效分析
Early Clinical Efficacy of Dexamethasone,Cerebroside Carnosine Combined with Hyperbaric Oxygen Chamber in the Treatment of Carbon Monoxide Poisoning
【摘要】 目的探讨地塞米松、脑苷肌肽联合高压氧舱治疗一氧化碳中毒患者的早期临床疗效。方法选取我院一氧化碳中毒患者42例,随机分为观察组和对照组,各21例。对照组给予高压氧舱、常规吸氧、改善循环、清除氧自由基、保护脑细胞及对症支持治疗,观察组在对照组的基础上给予地塞米松和脑苷肌肽。比较两组患者的临床疗效。结果观察组总有效率明显高于对照组(χ~2=4. 500,P<0.05);两组治疗前后血清CK、CK-MB水平均显著降低(P_均<0. 05),但观察组以上指标改善情况与对照组相比差异无统计学意义(P_均> 0.05);观察组治疗后简易智能精神状态量表(MMSE)总分明显高于对照组(P<0.05);观察组迟发性脑病发生率明显小于对照组(P<0.05)。结论地塞米松、脑苷肌肽联合高压氧舱治疗能明显改善一氧化碳中毒患者的早期临床症状、认知功能及神经功能缺损症状,减少迟发性脑病的发生率,临床疗效显著。
【Abstract】 Objective To explore the early clinical efficacy of dexamethasone,cerebroside carnosine combin-ed with hyperbaric oxygen chamber in the treatment of patients with carbon monoxide poisoning.Methods 42 patients with carbon monoxide poisoning in our hospital were randomly divided into observation group and control group,21 cases in each group. The control group was given hyperbaric oxygen chamber,conventional oxygen inhalation,impro-ved circulation,scavenging oxygen free radicals,protecting brain cells and symptomatic supporti-ve treatment. The observation group was given dexamethasone and cerebroside carnosine on the basis of the control group. The clinical efficacy of the two groups was compared.Results The total effective rate of the observation group was significantly higher than that of the control group( P<0. 05).Serum CK and CK-MB levels were significantly reduced before and after treatment in both groups( P<0. 05),but the improvement of the above indicators in the observation group was not statistically significant compared with the control group( P>0. 05).The total MMSE score of the observation group was significantly higher than that of the control group after treatment( P < 0. 05). The incidence of tardy encephalopathy in the observation group was significantly lower than that in the control group( P<0.05).Conclusion The combination of dexamethasone and cerebroside carnosine and hyperbaric oxygen chamber treatment can significantly improve the early clinical symptoms,cognitive function and neurological function defects of patients with carbon monoxide poisoning,reduce the incidence of delayed encephalopathy,and have a significant clinical effect.
