2型糖尿病肾病患者蛋白尿与血清TGF-β1、Klotho蛋白的关系探讨

【摘要】 目的研究2型糖尿病肾病(DN)患者蛋白尿与血清转化生长因子β1(TGF-β1)、单向跨膜(Klotho)蛋白含量的关系。方法选取2型糖尿病DN患者68例,其中无蛋白尿组(DM组) 23例,微量蛋白尿组(DN1组) 25例,大量蛋白尿组(DN2组) 20例,同期门诊诊治的单纯2型糖尿病患者22例为对照组(DC组),ELISA法检测患者血清Klotho蛋白、TGF-β1水平,统计并分析这两种蛋白与蛋白尿及临床资料的关系。结果 TGF-β1、Klotho蛋白水平与FPG无相关性(P>0.05),与Hb A1c、TG、UREA、CR和UACR有相关性(P<0.05); DM组、DN1组、DN2组TGF-β1水平高于DC组(P<0.05),Klotho蛋白水平低于DC组(P<0. 05); DN1组、DN2组TGF-β1水平高于DM组(P<0. 05),Klotho蛋白水平低于DM组(P<0. 05); DN2组TGF-β1水平高于DN1组(P<0. 05),Klotho蛋白水平低于DN1组(P<0. 05); 2型糖尿病DN中,TGF-β1与Klotho蛋白水平呈负相关(r=-0.528,P<0. 05)。结论 TGF-β1、Klotho蛋白水平可作为评估2型糖尿病蛋白尿严重程度及预后的重要指标。

【Abstract】 Objective To study the relationship between proteinuria,serum TGF-β1 and Klotho levels in patients with type 2 diabetic nephropathy( DN).Methods 68 patients with type 2 diabetic nephropathy in the First Affiliated Hospital of Jiamusi University were divided into three groups: no proteinuria group( DM,23 cases),microalbuminuria group( DN1,25 cases),and large proteinuria group( DN2,20 cases).22 patients with type 2 diabetes who were diagnosed and treated at the same time as the control group( DC group).The blood Klotho protein and TGF-β1 concentration were measured by ELISA,and the relationship between these two proteins and proteinuria and clinical data was analyzed.Results There was no correlation between TGF-β1,Klotho protein levels and fasting blood glucose( P > 0. 05),but correlated with Hb1 Ac,TG,UREA,CR and UACR( P<0.05).In type 2 diabetic nephropathy,TGF-β1 was a negative correlation between Klotho protein expression( r =-0.528,P<0.05).Conclusion When the patient’s serum TGF-β1 increased and Klotho protein decreased,it can be helpful for the diagnosis of type 2 diabetes proteinuria.The combined detection of serum TGF-β1 and Klotho protein levels in patients can be used as an important index to evaluate the severity and prognosis of type 2 diabetes proteinuria.
