CT薄层扫描数据辅助3D打印技术在闭合性跟骨骨折临床治疗中的应用价值探讨

【摘要】 目的探讨CT薄层扫描数据辅助3D打印技术在闭合性跟骨骨折临床治疗中的应用价值。方法选取2017年4月至2018年2月我院收治的闭合性跟骨骨折患者23例,采用CT薄层扫描数据辅助3D打印技术打印1:1比例实体模型,记录患者3D快速成型时间、临床资料与3D打印模型模拟手术一致性,评估患者手术后6个月时踝关节功能情况。结果 23例患者1:1骨折模型3D快速成型时间为2.3~4.9小时,平均骨折模型3D快速成型时间(3.36±0.36)小时;手术时间59~97min,平均手术时间(74.36±10.36)min。手术中实际应用螺钉长度及个数与3D打印模型模拟手术中一致,手术后,23例患者均未出现切口软组织感染、复位丢失等情况。手术后第6个月AOFAS评分优者14例(60.86%),良8例(34.78%),尚可1例(4.34%),23例患者整体踝关节功能恢复良好,优良率为95.65%。结论 CT薄层扫描数据辅助3D打印技术可帮助临床早期进行闭合性跟骨骨折手术模拟及方案制定,提高手术精度与安全性。

【Abstract】 Objective To explore the application value of 3 D printing technology assisted by data of CT thin-layer scanning in the clinical treatment of closed calcaneal fractures. Methods 23 patients with closed calcaneal fractures admitted to our hospital from April 2017 to February 2018 were selected. CT thin-layer scan data was used to assist the 3 D printing technology to print solid model with 1:1 scale. The 3 D rapid molding time and consistency between clinical data and simulated surgery of 3 D printed model of patients were recorded, and the function of ankle was evaluated after operation for 6 months. Results The 3 D rapid molding time of 23 patients in fracture model with 1:1 scale was 2.3~4.9 hours, and the average 3 D rapid molding time of fracture model was(3.36±0.36) hours. The operation time was 59~97 min, and the average operation time was(74.36±10.36) min. The length and number of screws used actually in the operation were consistent with those in the simulation of 3 D printed model. After the operation, 23 patients had no infection of incision in soft tissue and loss of reduction. After surgery for 6 month, the AOFAS score of 14 cases was excellent(60.86%), AOFAS score of 8 cases was good(34.78%), and AOFAS score of 1 case was passable(4.34%). The recovery of overall ankle was good in 23 patients, and the excellent and good rate was 95.65%. Conclusion The 3 D printing technology assisted by data of CT thin-layer scanning can help the early clinical simulation of closed calcaneal fractures and help make the plan to improve the accuracy and safety of surgery.
