【摘要】 目的探讨塑形性支气管炎(PB)患儿的临床特征以及白细胞介素(IL)-22、IL-6、肿瘤坏死因子-α(TNF-α)等炎性因子水平对病变的影响。方法选取茂名市妇幼保健院自2016年9月至2018年9月收治的54例PB患儿为PB组,另取急诊因异物吸入需行支气管镜异物取出术的15例患儿为普通组。酶联免疫吸附剂测定法检测支气管灌洗液中IL-22、IL-6和TNF-α水平。结果 54例PB患儿中肺不张29例(53.7%)、胸腔积液4例(7.4%)、肺气肿3例(5.6%)、纵隔移位2例(3.7%)。纤维支气管镜下均可见黏液栓阻塞支气管,病理学分型全部表现为Ⅰ型炎症,病原体以肺炎支原体为主,PB组患儿支气管灌洗液中IL-22显著低于普通组,IL-6和TNF-α显著高于普通组,差异均有统计学意义(P<0.05)。结论 PB患儿支气管灌洗液中IL-22显著减低,IL-6和TNF-α显著升高,上述3种炎症因子可能参与PB的致病过程。更多还原

【Abstract】 Objective To investigate the clinical characteristics of children with plastic bronchitis(PB) and the effect of interleukin(IL)-22,IL-6 and tumor necrosis factor-α(TNF-α) levels on the lesion.Methods A retrospective analysis of 54 cases PB children who were admitted from September 2016 to September 2018 was conducted.In the emergency department,15 cases requiring bronchoscopic foreign body removal due to foreign body inhalation were in the general group.The levels of IL-22,IL-6 and TNF-α inflammatory factors in bronchial lavage fluid were detected by ELISA.Results Among 54 PB patients,29 cases(53.7%) had atelectasis,4 cases(7.4%) had pleural effusion,3 cases(5.6%) had emphysema and 2 cases(3.7%) had mediastinal displacement.Bronchial obstruction with mucus embolism was seen under bronchofibroscopy.Pathological classification was inflammation(type I).Mycoplasma pneumoniae was the main pathogen.The IL-22 expression in PB group was significantly lower than that in the general group,while the expression of IL-6 and TNF-α in the PB group were significantly higher than those in the general group,with statistically significant difference(P<0.05).Conclusion IL-22 was significantly reduced and IL-6 and TNF-α are significantly increased in the broncholavage fluid of PB children.These three inflammatory factors may be involved in the pathogenesis of PB.更多还原
