【摘要】 目的探讨不同来源精子行卵胞浆内单精子注射(ICSI)治疗不射精症患者的临床疗效。方法回顾性分析2012年1月至2017年6月在天津市中心妇产科医院生殖医学中心行ICSI治疗的93例不射精症患者的临床资料,其中56例采用阴茎震动刺激法获得射出精液中的精子,37例采用睾丸精子抽吸术(TESA)获得的睾丸精子。比较两组间受精率,新鲜胚胎移植周期的着床率、临床妊娠率及活产率等情况。结果采用阴茎震动刺激法获得射出精子组和外科取精术获得的睾丸精子组的ICSI受精率分别为59.7%和60.2%,差异无统计学意义(P=0.89);优质胚胎形成率分别为35.9%和45.5%,新鲜胚胎移植周期的着床率分别为28.2%和48.6%,临床妊娠率分别为28.6%和51.4%,活产率分别为25.0%和45.9%,差异有统计学意义(P<0.05)。结论不射精症患者采用外科取精术获得的睾丸精子较阴茎震动刺激法获得射出精子行ICSI可获得更高的着床率、临床妊娠率及活产率。更多还原

【Abstract】 Objective To investigate the clinical effect of different sources of spermatozoa conducting intracytoplasmic sperm yeinjection(ICSI)for treating the patients with anejaculation.Methods The clinical data of 93 patients with anejaculation treated by ICSI in the Center for Reproductive Medicine of Tianjin Central Hospital of Obstetrics and Gynecology from January 2012 to June 2017 were retrospectively analyzed.Among them,56 cases obtained the ejaculated sperms by adopting the penis vibration stimulating(PVS)method and 37 cases obtained the testicular sperms by adopting the testicular sperm aspiration(TESA).The fertility rate,implantation rate of fresh embryo transfer cycle,clinical pregnancy rate and live birth rates were compared between the two groups.Results The fertilization rates of ICSI in the obtaining ejaculated sperms by PVS group and obtaining sperms by TESA group were 59.7%and 60.2%respectively,and the difference was not statistically significant(P=0.89);the good-quality embryo formation rates were 35.9% and 45.5% respectively,the implantation rates of fresh embryo transfer cycle were 28.2%and 48.6%,the clinical pregnancy rates were 28.6% and 51.4%,and the live birth rates were 25.0% and 45.9%respectively,and the differences were statistically significant(P<0.05).Conclusion Conducting ICSI with the testicular sperm by adopting the surgical sperm extraction in the patients with anejaculation can obtain higher implantation rates,clinical pregnant rate and live birth rate compared with obtaining the ejaculated sperms by PVS.更多还原
