2016—2017年滨州地区血流感染病原菌分布及其耐药性分析

【摘要】 目的了解滨州地区血流感染病原菌分布及其主要病原菌对抗菌药物的耐药情况。方法收集某院2016年1月—2017年12月住院患者送检血培养阳性且符合血流感染诊断的非重复标本。应用质谱仪、Vitek2 Compact全自动微生物鉴定及药敏仪对菌株进行鉴定和药敏分析,API系列手工鉴定和K-B法药敏试验作为补充,依据CLSI 2017年标准判定药敏试验结果;应用WHONET 5.6软件进行数据统计分析。结果共收集非重复病原菌975株,其中革兰阴性菌631株(64.7%),革兰阳性菌321株(33.0%),真菌13株(1.3%),厌氧菌10株(1.0%)。未检出碳青霉稀类耐药的肠杆菌科细菌(CRE),大肠埃希菌和肺炎克雷伯杆菌对哌拉西林他唑巴坦、头孢哌酮舒巴坦、阿米卡星的耐药率较低(<5%)。葡萄球菌属未发现对万古霉素、替考拉宁和利奈唑胺耐药的菌株,耐甲氧西林金黄色葡萄球菌(MRSA)和耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)的检出率分别占各自分离菌株的18.8%、72.8%。未检出对万古霉素耐药的肠球菌(VRE)。结论滨州地区住院患者血流感染以革兰阴性菌为主,菌种呈现多样性,对常用抗菌药物耐药情况不同,临床治疗应根据药敏试验结果合理选择抗菌药物。

【Abstract】 Objective To investigate the distribution and the resistance of major pathogens to antimicrobial agents with bloodstream infection from 2016 to 2017 in Binzhou.Methods Non-repeated specimens of blood culture positive were collected from inpatients of Binzhou Medical University Hospital with the diagnosis of bloodstream infection from January 2016 to December 2017.Identification and drug susceptibility analysis were adopted by mass spectrometer and Vitek2 Compact.API series of manual identification and K-B method were used as supplementary tests,and the durg susceptibility results were determined by CLSI standard 2017.WHONET5.6 software was used for data analysis.Results A total of 975 strains of non-repeated pathogens were collected,among which 631 gramnegtive bacteria (64.7%),321 gram-positive bacteria (33.0%),13 fungi (1.3%) and 10 anaerobic bacteria (1.0%).CRE were not detected.The resistant rates of E.coli and K.pneumonia to piperacillin-tazobactam,cefoperazone-sulbactam and amikacin were lower (< 5%).Strains resistant to vancomycin,teicoplanin and linezolid in Staphylococcus species were not found.The rates of MRSA and MRCNS were 18.8%,72.8% respectively.No VRE was detected.Conclusion The blood stream infection of inpatients in binzhou area was mainly caused by gram-negative bacteria,and the drug resistance to the common antibiotics were different.Antibiotic agents should be selected reasonably according to the results of drug susceptibility tests in clinical treatment.
