血浆髓过氧化物酶水平与冠状动脉狭窄程度的相关性分析

【摘要】 目的探究血浆髓过氧化物酶(myeloperoxidase,MPO)水平与冠状动脉狭窄程度的相关性。方法选取2013年7月至2015年12月在云南省楚雄彝族自治州人民医院心内科住院接受冠状动脉造影(coronary angiogram,CAG)的710例患者作为研究对象,分为冠状动脉综合症(arteria coronaria,ACS)组,稳定型心绞痛(stable Angina,SAP)组,冠状动脉造影结果正常人员作为对照组,观察血浆髄过氧化物酶水平与冠状动脉狭窄程度相关性。结果三组入选对象在高血压患者所占比重、血糖水平、尿素氮、肌酐等指标上进行对比差异均无统计学意义(P> 0. 05); ACS组与其他两组MPO水平对比差异较为明显,具有统计学意义(P<0.05);SAP与对照组在MPO水平上进行对比,差异无统计学意义(P>0.05);无狭窄者MPO水平为14.25±3.26μg/L,明显低于其他组患者,差异具有统计学意义(P <0.05);重度狭窄患者MPO水平为41.58±10. 64μg/L,明显高于其他组患者,差异具有统计学意义(P <0.05);血浆MPO水平与冠状动脉狭窄程度呈正相关关系(r=0.534,P=0.235)。结论冠状动脉狭窄程度越高则相应的血浆MPO水平越高,可将血浆髓过氧化物酶作为冠状动脉病变的辅助诊断指标。

【Abstract】 Objective To investigate the relationship between plasma myeloperoxidase(MPO) and coronary artery stenosis. Methods From July,2013 to December,2015,710 patients who received coronary angiography(coronary angiogram, CAG) in Department of Cardiology, Chuxiong Yi Autonomous Hospital, Yunnan Province, were divided into the coronary artery syndrome(arteria coronaria, ACS) group and the stable angina pectoris(Stable Angina,SAP) group. The plasma myeloperoxidase level and the degree of coronary artery stenosis were observed in both groups,as well in selected normal subjects as the control group. Results We selected portions of patients based on parameters,including hypertension,blood sugar level and urea in the three groups. There were no significant differences in nitrogen,creatinine and other indexes(all P >0.05).There was significant difference in MPO level between the ACS group and the other two groups(P <0.05),but no difference between the SAP group and the control group(P >0. 05). The level of MPO in patients without stenosis was 14. 25 ±3. 26μg/L,which was significantly lower than that other groups(P <0.05),and the level of MPO in patients with severe stenosis was 41.58 ± 10.64μg/L,which was significantly lower than that in other groups(P < 0. 05). The plasma MPO concentration was positively correlated with the degree of coronary artery stenosis. Conclusion The higher the degrees of coronary artery stenosis are, the higher the corresponding plasma MPO levels are, which could serve as an diagnostic marker for coronary artery stenosis.
